An exciting opportunity has arisen to join the Falls Prevention Team within the Livewell Wellbeing services.

You will contribute to reducing falls and fall-related injuries across Plymouth, supporting older adults to live their best lives; utilising behaviour change skills to support engagement and longer-term adherence to physical activity and specific falls prevention programmes.

Under the guidance of physiotherapists and health improvement practitioners you will be integral in supporting delivery of falls prevention interventions; implementing rehabilitation plans, practicing with equipment, and supporting exercise, both on a 1:1 basis in patients homes or clinics and in our group-based programme, FaME (Falls Management Exercise), within community venues across Plymouth.

This role is a 6 month fixed term contract, up to 22.5 hours per week.

Main duties of the job
The Wellbeing Team supports delivery of falls prevention activities. The falls prevention team are responsible for carrying out multifactorial falls risk assessments and interventions for adults over the age of 55 who are at risk of falls or who have fallen. This includes the delivery of group-based strength and balance classes - Falls Management Exercise (FaME) and home-based exercise delivery, including Otago. The team also plays a preventative role by spreading key messages, for example, the importance of Moving More, strength and balance exercise, bone health and other evidence-based falls prevention activities, through health promotion events across the city.

The post holder will be responsible for the provision of rehabilitation of individuals who have a wide range of long-term and complex conditions. Under the guidance of registered physiotherapists and health improvement practitioners, the post holders role will include supporting the delivery of evidence-based falls prevention interventions, including FaME group delivery, implementing rehabilitation plans and empowering individuals to achieve their agreed goals. This may include, practicing with equipment, mobility practice and supporting exercise plans, underpinned by effective use of behaviour change conversations.

Job responsibilities
Clinical Responsibilities
To ensure that individuals are fully informed and consent to rehabilitation.

Manages delegated caseload with support from the registered practitioner.

To implement rehabilitation programmes for those individuals triaged direct to the support worker while following the service protocols.

To meet regularly with the relevant therapist to discuss treatment goals, progress and moving on from the service.

Communicate outcomes of intervention to the referrer, GP and individual receiving rehabilitation via a discharge report following consultation with the registered practitioner.

To assist therapists/practitioners in setting up/arranging assessments and rehabilitation sessions, this may include both standardised and non-standardised assessments, in the department, a community venue or on a home visit.

As part of the intervention, you are required to liaise with family/carers, departments and organisations following consultation with the registered practitioner.

Carry out observations and rehabilitation, following the guidance given by the therapists, whilst maintaining the persons privacy and dignity. This may include teaching new techniques and/or teaching the use of equipment.

Use own judgement to vary and adapt choice of activity to fit the individuals needs and capabilities to achieve treatment goals. Feeding back any changes to the identified registered practitioner.

To assess for, demonstrate and instruct in the safe use and maintenance of rehabilitative equipment as prescribed by the therapist or yourself. Order the equipment in line with the CES policy and service guidelines.

Maintain high standards of care at all times in a manner sensitive to the individuals needs, acknowledging and respecting their rights, individual values, cultural and religious diversity at all times.

Contribute to the evaluation process through knowledge of simple rehabilitation evaluation measures and tools.

Participate in the management of a waiting list of referrals.

Manage your own diary of appointments.

To work jointly with other health, social care and wellbeing team colleagues.

Responsibility for Communication
To record and report regularly on an individuals progress and treatment methods to the MDT or registered practitioner in accordance with the policies and procedures of the team.

To use a range of verbal and non-verbal communication methods with individuals who may have difficulty understanding or communicating and who may be low in mood and lack of insight.

To receive sensitive information, for e.g., regarding an individual you are working with, or their carer and communicate this information to the supervising therapist and relevant members of the MDT.

Maintain confidentiality and security of patient records in line with the Trust policy and procedure.

To communicate effectively by phone, fax and electronically with a wide variety of departments/organisations both internally and externally to the Trust.

To be responsible for record keeping in line with the Trust, Professional policies and procedures.
