Main duties of the job
An excellent opportunity has arisen for a Health Practitioner to work within the Calderdale 0-19 (25) services based in the Calderdale Family Hubs Family Connect Team.

The Family Hubs Health Practitioner (FHHP) will work within a multidisciplinary team and will act as the first point of contact for those reporting the concerns of children and young people, and families in Calderdale.

• You will have a sound working knowledge of health services that support the family.
• You will deliver support and guidance to families and professionals in an engaging, caring and non-judgmental manner.
• You will practice professional curiosity when handling calls to achieve a desired outcome, safely and effectively.
• You will handle information in confidence, which at times can be emotive and sensitive in nature.
• You will play a central role in embedding a "whole family" approach in line with the early help strategy, working collaboratively across local authority, health, education, and voluntary sector partners, with an aim to reduce escalation to statutory services, through early identification, intervention, and robust multi-agency planning.

This role is not applicable for sponsorship and candidates must be able to demonstrate their right to work in the UK.
