Job summary
We are looking for an experienced, motivated Band 6 Registered Nurse to join our supportive and dynamic team as a Shift Co-ordinator and Triage Clinician within MFT's Clinical Single Point of Access (SPOA).

This is an exciting opportunity for an experienced clinician who is confident in clinical assessment, decision-making and care co-ordination, and who is passionate about delivering high-quality, person-centred care across the community.

As part of our Community Urgent Care Services. The service works collaboratively across a range of integrated pathways across all of our urgent care and specialist teams.

As a Shift Co-ordinator and Triage Clinician, you will undertake clinical triage and assessment, prioritise referrals and ensure patients are directed to the most appropriate service or pathway. You will work closely with colleagues across the multidisciplinary and multi-agency teams to facilitate safe, timely and effective care.

Main duties of the job
The post holder will work as part of the Clinical Single Point of Access (SPOA) team, providing expert clinical triage, assessment and co-ordination to ensure patients are referred to the most appropriate service and receive timely, safe and effective care. The post holder will support the shift co-ordination function and work collaboratively with colleagues to manage referrals, prioritise clinical need and facilitate appropriate onward referral, intervention or discharge.

The post holder will work as part of a multidisciplinary and multi-agency team, collaborating closely with hospital staff, community teams, primary care and other health and social care services to facilitate timely access to appropriate care. Through comprehensive clinical assessment, risk management, care planning and effective communication, the post holder will support integrated working across the acute and community interface and help prevent avoidable hospital admissions wherever appropriate.

The post holder will provide and/or arrange appropriate clinical support, interventions and onward referrals within the community, working closely with colleagues across all sectors to ensure continuity and coordination of care. They will use their specialist knowledge and clinical skills to provide high-quality, safe and effective clinical advice and decision-making within the SPOA and across the wider Community Urgent Care Services.
