Mansion House Surgery is looking to recruit a Clinical Pharmacist to join our forward-thinking practice team. The Practice team comprises ANPs, MH nurses, Clinical Pharmacist, First Contact Physios and Paramedics.

Main duties of the job
Core responsibilities will include:

• Medication reviews including: Structured medication review, Anticoagulant Annual Reviews, Care home medication reviews and Medication reviews from notes where appropriate
• Medicines reconciliation including hospital discharge, clinic letters and new patient registrations, working alongside the Pharmacy & wider practice team to resolve queries where needed.
• Condition focused clinics depending on candidate specialisms/experience as fits practice need

Mansion House Surgery is based on the canal side in the market town of Stone in a modern, purpose built premises with a large car park.

The practice is actively involved with the Stone and Eccleshall PCN and is a training and research practice.

Job responsibilities
The Clinical Pharmacist will play a key role. Working as part of a multidisciplinary team, the post holder will contribute through medicines optimisation, structured medication reviews, and proactive management of longterm conditions. The role will focus on improving patient outcomes, reducing clinical risk, and supporting preventative care initiatives, including cardiovascular health improvement. This position offers the opportunity to work autonomously while collaborating closely with GPs, nurses, and wider colleagues to enhance the safety, quality, and efficiency of prescribing within the practice.

This will include but not be limited to the following:

• Providing specialist medicines management advice to GPs, practice nurses, community pharmacists, community nurses and other healthcare professionals to support highquality, evidencebased, and costeffective prescribing.
• Supporting the practice in achieving PCN DES IIF and QIF targets.
• Leading and promoting quality improvements in medicines management across the practice, ensuring best practice is consistently adopted.
• Working closely with pharmacy technicians to deliver medicines management objectives and enhance workflow efficiency.
• Strengthening pharmaceutical knowledge and understanding across the wider primary care team.
• Undertaking risk assessment and management activities, ensuring compliance with medicines legislation and promoting safe systems of work.

Person Specification
Qualifications
Essential

• Master's degree in Pharmacy (MPharm) or equivalent(E)
• Registration with the General Pharmaceutical Council (GPhC) (E)
• Independent Prescribing Qualification (E) (with prescribing experience)

Experience

Skills
Essential

• Strong therapeutic and clinical knowledge, with understanding of evidence based healthcare
• Understanding of primary care prescribing, rational prescribing principles and medicines optimisation strategies
• Ability to plan, manage, monitor and review medicines optimisation activities, particularly for long term conditions
• Proficient IT skills, including Microsoft Office, and ability to adapt to new clinical systems
• Ability to analyse and interpret complex clinical and technical information
• Strong problem solving skills, with the ability to recognise priorities and escalate appropriately
• Ability to work under pressure, manage competing deadlines and produce timely, accurate reports
• Ability to motivate, influence and gain acceptance for recommendations, even where barriers exist
• Ability to work independently and collaboratively within a multi disciplinary team
• Demonstrates accountability for delivering professional expertise and direct patient facing services
