This is a combination of visiting patients in their home with site attendance required as part of the role. This is far more than a traditional clinical role, as an autonomous practitioner, you will assess, diagnose, treat, and manage patients presenting with a wide range of urgent and acute conditions while working collaboratively with multidisciplinary teams to deliver exceptional patient care. Alongside providing expert clinical care, you will play an active role in service development, quality improvement, education, and the implementation of innovative ways of working that enhance patient outcomes across Primary Care. Main duties of the jobThis is a dynamic advanced clinical role requiring excellent assessment, diagnostic, and decision-making skills. You will provide safe, evidence-based, person-centred care while working autonomously and collaboratively across multidisciplinary teams. You will assess patients presenting with urgent and acute conditions, formulate personalised management plans, prescribe where appropriate, and ensure patients receive the right care at the right time. Alongside clinical practice, you will contribute to service development, quality improvement initiatives, clinical leadership, and the continuous improvement of patient care across the division. You will need excellent clinical judgement, adaptability, and a commitment to delivering outstanding patient care. Job responsibilitiesKey responsibilities include:
- Undertaking comprehensive clinical assessments, diagnosis, treatment, and management of patients presenting with urgent healthcare needs
- Developing evidence-based, personalised care plans in partnership with patients, carers, families, and healthcare professionals
- Working autonomously to make safe clinical decisions within your scope of practice
- Prescribing safely and appropriately where qualified to do so
- Managing minor illness, long-term conditions, and complex patient presentations
- Delivering telephone triage, face-to-face consultations, and home visits where required
- Working collaboratively with GPs, nurses, allied health professionals, and external healthcare providers
- Supporting clinical governance, audit, service evaluation, and quality improvement initiatives
- Contributing to education, supervision, mentoring, and the development of colleagues and learners
- Supporting the implementation of innovative clinical pathways and new models of care across the organisation
Person SpecificationQualificationsEssential- Registered General Nurse or Registered paramedic
- Recognised degree qualification in autonomous practice
- Independent prescribing is advantageous
- MSc or equivalent (relevant to the position) is advantageous
