Job summary
We are seeking registered nurses to join our All Age Continuing Care team as AACC Nurse Assessors. This developmental Band 5 role offers the opportunity to build specialist skills in NHS Continuing Healthcare and NHS Funded Nursing Care while making a direct difference to people with complex health and care needs. You will undertake defined review and triage activity within a clear scope of practice, supported by senior clinicians, structured supervision, training and competency development. We are looking for compassionate, organised nurses who communicate well, work confidently within agreed parameters and recognise when to seek advice or escalate concerns.

This role will require weekly office-based working in line with the hybrid working policies set out for each ICB, with office bases available in Coventry, Warwick, Worcester and Hereford for all staff to use. While this role is expected to be place-based, travel may be required across the cluster and to care settings as needed.

NHS Coventry & Warwickshire ICB and NHS Herefordshire & Worcestershire ICB are working together as a cluster with a shared management team. Main duties of the job
You will undertake delegated 1:1 care package reviews, non-complex 12-month CHC reviews, FNC assessments and reviews, and Fast Track triage or pre-review activity across home, community, residential and nursing care settings. You will gather and assess clinical information, maintain accurate records, contribute to person-centred decision-making and manage a defined workload within agreed criteria.

You will work with individuals, families, carers, multidisciplinary teams, care providers and partner organisations. Cases involving significant deterioration, safeguarding concerns, disputes, high-cost package changes, legal challenge or potential eligibility decisions must be escalated to a Senior Nurse Assessor or Clinical Team Lead. You will be supported to develop your knowledge of CHC and FNC through supervision, training and competency assessment.

Assessment and review activity

• Undertake delegated 1:1 care package reviews, non-complex 12-month CHC reviews, and FNC assessments and reviews across community, home, residential and nursing care settings.
• Undertake Fast Track triage and pre-review activity, including gathering clinical information, checking the completeness of evidence, identifying urgency and escalating cases requiring senior clinical review.
• Apply the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care to support holistic, person-centred and evidence-based review processes.
• Maintain accurate, clear and timely records to support decision-making.

Scope of practice and escalation

• Work within a defined Band 5 scope of practice under the supervision of senior registered clinicians and within agreed protocols, competency frameworks and local procedures.
• Identify and promptly escalate significant clinical deterioration, safeguarding concerns, disputed or high-risk cases, high-cost package changes, legal challenges and matters outside agreed parameters.
• Participate in regular supervision, case discussion and competency review.

Working with people and partners

• Communicate clearly, compassionately and professionally with individuals, families and carers, ensuring their views and preferences are captured and respected.
• Work collaboratively with health, social care and provider organisations, and contribute to multidisciplinary reviews and meetings.
• Recognise and escalate safeguarding and quality concerns and comply with clinical governance, confidentiality and information governance requirements.

Professional development

• Complete local competency requirements before undertaking activity without direct oversight.
• Maintain current NMC registration, uphold the NMC Code and engage in continuing professional development and reflective practice.

Qualifications
Essential

• Registered Nurse with current NMC registration; newly qualified nurses may be considered where robust preceptorship, supervision and competency development arrangements are in place.
• Awareness of the National Framework for NHS Continuing Healthcare and Funded Nursing Care
