Job responsibilitiesThe RoleThis practice-based role is for 22.5 hours per week.
Your week will be varied and typically split between:
Planned Clinical Sessions- Structured Medication Reviews (SMRs).
- Long-term condition management.
- Patient-initiated medication reviews.
- Medicines optimisation.
Duty Pharmacist Sessions- Reviewing acute prescription requests.
- Responding to medication queries from patients and clinicians.
- Supporting safe prescribing across the practice.
- Working collaboratively with colleagues to resolve medication-related issues.
You'll receive regular support from our Lead Pharmacist and GP Clinical Supervisor, with dedicated pharmacy and practice meetings for case discussion, learning and professional development.
Main Duties- Provide expert medicines advice to the multidisciplinary team.
- Conduct Structured Medication Reviews and medication optimisation reviews.
- Manage patients with long-term conditions including:
Atrial Fibrillation
Chronic Kidney Disease
Heart Failure
Gout
Hypothyroidism
Hypertension
- Support nursing colleagues with the management of diabetes and respiratory conditions.
- Review acute prescription requests.
- Support medicines reconciliation following hospital admissions and outpatient appointments.
- Help the practice achieve local and national prescribing quality targets.
- Undertake medicines optimisation and quality improvement projects.
- Support vulnerable patients and help reduce barriers to accessing health care and medication.
- Maintain awareness of CQC and QOF medication safety requirements.
Person SpecificationKnowledge and SkillsEssential- Strong clinical knowledge and decision-making skills.

- Enjoys managing complex patients and long-term conditions.
- Works collaboratively within a multidisciplinary team.
- Is proactive, compassionate and motivated to improve patient outcomes.
- Wants to continue developing professionally within primary care.
QualificationsEssential- MPharm Degree.
- Ongoing evidence of Continuing Professional Development (CPD).
- Completed, or willing to undertake, the CPPE Primary Care Pharmacy Education Pathway.
Desirable- Independent Prescribing Qualification.
