We are recruiting to our new Intensive & Assertive Team, launching across Norfolk and Suffolk to provide intensive, flexible and recovery-focused support for people living with severe and enduring mental illness. This is a unique opportunity to join a service from the outset and help shape its future.

As a Care Coordinator, you will work with people who often find it difficult to engage with traditional mental health services, delivering assertive outreach, intensive community support and recovery-focused interventions. You will help improve engagement, reduce hospital admissions and support individuals to live more fulfilling lives within their communities.

Posts Available and Locality Working
We currently have 12.5 WTE Care Coordinator vacancies across Norfolk and Suffolk. Successful applicants will have the opportunity to discuss their preferred locality, and wherever possible we will align staff to their chosen area to minimise travel and support strong local partnerships.

As a county-wide service, there may occasionally be a requirement to support colleagues and service users across your allocated county in response to service needs. Any wider working will be reasonable, proportionate and fully supported.

The team operates 7 days a week, 11:00am-7:00pm, with staff participating in a rota that includes weekends and bank holidays. We value flexibility, teamwork and work-life balance, and offer the chance to be part of an ambitious and innovative new service from day one.

Main duties of the job
Main Duties of the Role

• Provide care coordination to individuals with severe and enduring mental illness.
• Undertake comprehensive mental health, risk and psychosocial assessments.
• Develop, implement and review co-produced care, recovery and crisis plans.
• Deliver evidence-based interventions in accordance with national guidance and Trust policies.
• Work proactively with individuals who may struggle to engage with traditional services.
• Deliver assertive outreach interventions within community settings, including service users' homes.
• Promote recovery, social inclusion and independence.
• Support individuals to access physical healthcare, housing, welfare benefits, employment, education and community resources.
• Undertake risk assessment and formulate effective risk management plans.
• Respond to crises and support admission avoidance where appropriate.
• Work collaboratively within a shared caseload model.
• Develop effective partnerships with carers, families and external agencies.
• Maintain accurate and timely clinical documentation.
• Contribute to service development, quality improvement and clinical audit activity.

Person Specification
Qualifications
Essential

• Registered Mental Health or Learning Disability Nurse, Occupational Therapist OR Social Worker with the relevant professional registration
• Practice Assessor and Practice Supervisor training (NMC registrants) or Practice Educator (AHP & Social Worker Registrants)
• Ongoing professional development working towards degree level ( if professional registration is following a diploma level course)

Desirable

• Higher degree or post graduate certificate in area of clinical / therapeutic practice

Experience
Essential

• Able to demonstrate an appropriate level of knowledge of mental health practice from previous experience
• Experience of managing a caseload and the provision of Mental Health Care services to people with mental health care needs
• Experience in working in team delivering person centred treatment packages

Desirable

• Experience in delivering group based interventions Experience in supporting service development and quality improvement initiatives

• Ability and willingness to travel independently to meet the needs and requirements of the service
• Undertake physical intervention training and remain trained in techniques, as required by role
