Job Summary
You will be joining Haringey Community Rehabilitation Team as a Band 6 care coordinator. The team promotes Trauma Informed Rehabilitation based recovery work to support of service user independence.

You will be working with service users with complex psychosis that reside in 24 hour supported accommodation and will have a complex caseload.

You will be joining a very supportive team, where you can pull on the experience and expertise of the full MDT.

Main duties of the job
  • Carry overall responsibility for the management of a caseload, acting as a care coordinator.
  • Deliver high quality assessment, effective care and treatment that is carried out within a multidisciplinary framework and in collaboration with other agencies.
  • Involve service users and their carers in planning care that includes all of their individual and diverse needs.
  • Design individual programmes of care collaboratively with patients to maximize self management and social inclusion and reflect their own goals and aspirations. Review and adjust plans according to changing service user needs.
  • Provide evidence based therapeutic interventions using developed interpersonal skills such as CBT or psycho-education.
  • Provide service users and their carers with clear and intelligible information which will assist them in making informed decisions about their treatment and care.
  • Administer medication, monitor for effects and side-effects, in accordance with NMC Standards and Trust policies.
  • Facilitate vocational rehabilitation and those aspects of mental health recovery concerning occupation.
  • Maintain clear, comprehensive and contemporaneous clinical records on all patients in response to personally generated clinical observations, in accordance with professional standards and Trust policies.

In order to meet the needs of the Trust you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location that fall under Barnet, Camden, Enfield, Haringey or Islington. The Trust reserves the right to require staff to work at such other places or locations as it considers reasonable and necessary on a temporary or permanent basis.

Person Specification
Qualifications
Essential
  • Registered Mental Health Nurse

Desirable
  • Relevant qualification in Psychosocial Interventions with people with severe and enduring mental health problems.

Skills/Ability
Essential
  • Significant years post registration experience, at least one of which will be working with people with severe and complex mental health problems.

Desirable
  • Experience of teaching, training and/or mentoring.

Experience/Knowledge
Essential
  • Understanding of current issues informing the Health Agenda.

Desirable
  • Ability to motivate and develop staff.
