Job summary
Two full-time roles available - one in Haringey East Core Community Team and one in Haringey Central-West Core Community Team

As a Key Worker Practitioner, the post holder will be responsible for coordinating a variety of multi-disciplinary interventions as part of each service user's treatment journey. They will act as the primary point of contact for service users, carers, and other professionals involved in the individual's care.

This role requires flexibility and the ability to respond to the evolving needs of the service and its users. The post holder may be assigned additional duties by the Team Manager in line with their competencies and experience, to support the effective operation of the service.

Main duties of the job
Leading referrals to appropriate services and support systems based on team discussions and treatment planning.

Developing, implementing, and monitoring care plans and discharge plans under the supervision of the Team Manager or designated clinical lead.

Delivering care in line with NICE guidelines, evidence-based practice, and principles of recovery and reablement.

Maintaining responsibility for a defined caseload and contributing to the ongoing development of person-centred care and treatment plans for individuals with mental health needs.

In order to meet the needs of the Trust you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location that fall under Barnet, Camden, Enfield, Haringey or Islington. The Trust reserves the right to require staff to work at such other places or locations as it considers reasonable and necessary on a temporary or permanent basis.

Job responsibilities
  • To maintain up to date patient records to the standard required by the Trust
  • II. To follow Trust policy in promptly reporting all accidents and incidents and contribute to undertaking complaints or investigations if required.
  • III. To support other members of staff in their assessment and management of service user care needs through reviewing initial data collection; monitoring their progress; feedback; discussion and reports at care reviews.
  • To collect and record clinical or activity information as required by IT data bases for the Trust and Adult Social Care and carry out any other administrative duties as required by the team.
  • To undertake a range of necessary tasks, reporting to the Team Manager or designated supervisor. This will include participating and contributing to initial assessment and formulation, developing and agreeing care plans with service users, supporting service users to understand their treatment plans, implementing care plans in conjunction with the wider multi-disciplinary team, ongoing review and evaluation of an individuals care, and identifying and referring on to appropriate support services for ongoing care. This will involve delivering care and interventions appropriate to the post holders level of experience and competencies, without direct supervision.
  • To contribute to the multi-disciplinary team comprehensive, culturally appropriate assessments and to multi-disciplinary team decisions at the point of discharge and/or transfer of the service user.
  • To participate in and provide own perspective to multi-disciplinary team meetings/case discussions.

Person Specification
Qualifications
Essential
  • Registered mental health professional qualification or Nationally recognised qualifications in relevant/identified posts (teaching, City and Guilds, sports coaching) or Equivalent level of experience
  • Current Level 1 Safeguarding Children and Safeguarding Vulnerable Adults certificates or willingness to undertake

Desirable
  • Evidence of recent and ongoing academic study
  • Level 2 Safeguarding Children or Safeguarding Vulnerable Adults certificate
  • Mentorship or recognised teaching and coaching certificate

Skills/Abilities
Essential
  • Excellent verbal and written communication skills
  • Able to work on own initiative
  • Able to work on own initiative, motivated and enthusiastic
  • Competent using computers and keyboards for entering personally generated data, generating and responding to emails, conducting web searches etc.
  • Demonstrate and understand the recovery philosophy
  • Demonstrate well developed interpersonal skills
  • Ability to maintain boundaries and form positive therapeutic relationships with service users and carers
  • Sound awareness of patient / service user care within the wider context of the organisation
  • Demonstrate a clear understanding of the roles within the multi-professional team

Experience/Knowledge
Essential
  • Demonstrable experience of caring for people with mental health issues in an in-patient, community or substance misuse care setting
  • Demonstrable experience of working in a multi-partnership approach
  • Demonstrate a good understanding of the biopsychosocial factors that could have an impact on an individual's wellbeing and preferred treatment methods
  • Experience of or, if appropriate to service needs, be willing to undertake training in de-escalation or breakaway techniques.

Desirable
  • Demonstrate a clear understanding of current Mental Health legislation and its application / use in practice or legislation relevant to services.
  • Practical experience of mentoring or supervising students, other trainees or new staff
  • An awareness of current research and evidence-based practice within the setting
  • Demonstrate an understanding of the management of the physical health needs of people with mental health or substance addictions problems.
