Job Summary / Role:
To support the Band 7 ILD Clinical Nurse Specialist with the delivery of comprehensive, multidisciplinary management of ILD focusing on improving patient outcomes through specialized care, education, and support to minimize the impact of ILD on daily life.

To be a member of, actively participate in, and work within the guidelines of the relevant MDT

To co-ordinate the care throughout the patients pathway whilst under the care of the MDT, from diagnosis through treatment and follow-up care

To provide appropriate written information for patients, relatives and hospital staff covering relevant condition, its treatment.

To provide psychological, social and cultural support to patients diagnosed with ILD

To collaborate with Rheumatology services in the development of ILD services and delivery of joint clinics (to be held on all hospital sites).

To support the development of an ANCA associated vasculitis pathway in collaboration with renal services.

The post holder will fulfil all tasks and work as part of a team. To meet the needs of the service, the post holder may be required to work in other areas as appropriate as directed by the line manager.

Core Functions:
We are looking for an experienced registered nurse with excellent communication and organisational skills. The applicant should have experience in the management of respiratory patients and be able to provide evidence of this.

To support patients in meeting their own health and wellbeing through providing expert information, advice and support

To assess patients and their complex needs and those of their families and plan, implement and evaluate appropriate programmes of care this will include communicating highly sensitive information about diagnosis, treatment options and issues surrounding terminal illness and bereavement.

To provide emotional, psychological and practical support to the patient and their family/carer throughout their pathway and to facilitate communication between patients, families and professionals

To recognise ethical dilemmas relating to care and act as the patient/relatives advocate when required

To support the development of care pathways for patients with ILD

To provide nurse specialist support to ILD MDT clinics across UHT

The post holder will ensure patients have prompt access to specialist advice and giving high standards of evidence-based care, intervention and support at all times. Areas include full assessment of patients on acute medical wards and out-patient clinics.

The post holder will monitor medication, assess compliance with and impact of treatment plans, and arrange oxygen therapy if required.

Administrative Responsibilities
Manage own workload effectively, prioritising and using time management skills to ensure information is timely and accurate.

Keeps accurate and complete records consistent with legislation, professional guidelines, trust policies and procedures.

Promoting and maintaining confidentiality sharing accurate and timely information in line with the information governance agenda.

Ensure clear concise documentation of consultation

Provide patient/relative education regarding the nature and purpose of ILD.

Provide ongoing education and support to patients receiving treatment for ILD

Where appropriate, assess for and order home oxygen therapy including long term oxygen therapy and nocturnal oxygen therapy.

Clinical Responsibilities

To involve patients and carers in the planning of their care and treatment, ensuring appropriate consent and agreement is gained.

Considers and interprets all available information and makes justifiable assessment of the individuals health and well-being, related needs and risks explaining the possible outcomes to those concerned.

Develops and records care plans that are appropriate to the patient based on their assessment of the patient.

Ensures that nursing care is delivered with compassion.

Ensure documentation is completed in accordance with the NMC professional guidelines and trust policy.

Acts within own level of competency to undertake specific interventions and treatments seeking advice and supervision and training where appropriate.

Monitors patients response and reaction to intervention/treatment making clear decisions and taking appropriate action.

Provides feedback to the clinician responsible to the overall effectiveness.

Communicates and supports patients and their carers in understanding their care and treatment. Identifying potentially difficult and challenging situations and seeking appropriate and timely advice as required.

Liaises and communicates effectively with a range of people on a range of matters within the ILD MDT. Effectively using different methods of communication to support delivery of care.

Liaise with home oxygen service provider with regard to equipment and patients prescriptions.

To liaise with other clinical teams to provide appropriate and timely advice about ILD
