Job descriptionJob responsibilitiesClinical Care:- Provide advanced clinical assessment, diagnosis and management of adults with complex, long-term conditions - including frail, elderly and housebound patients - in their homes, care homes and community healthcare settings.
- Deliver proactive clinical care, including falls risk assessment, polypharmacy review, and anticipatory care planning.
- Work closely with families, carers and social care teams to support older patients with complex needs.
Leadership & Coordination:- Lead and coordinate clinical care pathways with GPs, community nursing, care homes, therapy teams, social care and voluntary organisations.
- Support the development of proactive frailty identification and early intervention within the practice and PCN.

Person SpecificationQualificationsEssential- Registered Nurse with valid NMC registration
- Independent Nurse Prescriber (V300)
ExperienceEssential- Significant post-registration experience in community or primary care, managing complex caseloads
- Experience of multi-agency, multidisciplinary working
