Job summaryThe Staff Nurse will deliver high quality, personalised, compassionate care to service users, providing a range of specialist interventions.
Ensure consistently high standards of practice and clinical excellence, which comply with the Nursing and Midwifery Council (NMC) code of professional conduct.
Lead the delivery of comprehensive service user assessment, planning, implementing and evaluating programmes of patient-centred care in a range of settings.
Provide clinical supervision and support to junior colleagues, including students.
Assist and support the manager, as directed, to deliver a nurse-led integrated care service and develop a specialist practice-based clinical model of care.
Maintain a safe and therapeutic working environment, working collaboratively with the Multi-Disciplinary Team and maintaining accurate records.
Band 5 Nursing roles are only open to staff with a Nursing registration, obtained as a result of completing a qualification recognised by the Nursing and Midwifery Council (NMC).

Job responsibilitiesKey Task and ResponsibilitiesExercise a high degree of personal and professional autonomy and have the ability to plan and reach complex and critical judgments, using decision-making skills.
Deliver high-quality clinical care within the service. This will include acute and primary care, long-term conditions management, enhanced assessment and diagnostic intervention services, health promotion activities and be in line with the NHS Plan, public health indicators and National Service Frameworks.
Demonstrate and provide robust clinical skills with a sound understanding of evidence-based nursing practice to provide a pro-active approach to ensure quality and outcome-driven practice on a day-to-day basis.
Support the development of evidence-based practice in the specialist field and promote research as appropriate.
Participate in managerial and professional clinical supervision, training, and annual appraisal and ensure that this becomes an integral part of team practice.
Ensure the implementation of effective and appropriate patient-centred care-planning tools, long-term condition registers and consistent delivery throughout the service.
Work collaboratively with the Clinical Lead, GPs, and other clinicians to meet and review service needs as required.
Be guided by standard operating procedures (SOPs), good practice, established precedents and understand what results or standards are to be achieved. Make comments on proposals for changes, regarding working practices, service improvements or patient feedback.
Work independently within organisational and professional guidelines and established professional parameters, working to the nationally recognised code of conduct and Standards of Proficiency for Nurses. Work as part of the multidisciplinary team.
Communicate effectively, with sensitivity and compassion, and manage relationships with people, making reasonable adjustments where necessary.
Ensure the privacy, dignity and safety of individuals is maintained at all times.
Monitor the condition and health needs of people within their care on a continual basis in partnership with the members of the MDT team, families and carers.
Person SpecificationEducationEssential- NMC Registered nurse with children's experience, or a registered children's nurse.
Desirable- Evidence of CPD related to a specialist children's nursing subject.
ExperienceEssential- Experience of working with children and young people in the community. This could be as a student, or pre-registration.
- Experience of working with children and young people with complex medical needs. This could be as a student.
Desirable- Knowledge of common bowel and bladder issues in children
KnowledgeEssential- An understanding of basic bladder and bowel management.
SkillsEssential- Clinical skills in history taking and assessment.
Desirable- Communication skills in the community.
