This Band 7 development role offers the opportunity to undertake postgraduate training in Advanced Clinical Practice while building advanced knowledge and skills across clinical practice, leadership, education and research, with a minimum of 20% off-the-job learning.

You will work semi-autonomously, supported by clinical and academic assessors, to deliver safe, evidence-based and person-centred care to a diverse patient population. Main duties of the job
As a Trainee Advanced Clinical Practitioner, you will undertake comprehensive, person-centred clinical and risk assessments, drawing on biological, psychological, social and spiritual perspectives. You will develop advanced clinical reasoning and decision-making skills to manage complex, incomplete or conflicting information, and plan, deliver and evaluate evidence-based care and treatment within your professional scope of practice.

You will recognise deterioration, prioritise interventions and escalate or refer appropriately to other professionals and services, while maintaining accurate, contemporaneous clinical records and care plans in line with governance requirements.

The role will also involve providing clinical leadership, supervision, teaching and support to colleagues, students and other healthcare professionals; contributing to clinical audit, research, quality improvement and service development initiatives; and working collaboratively across local care systems, partner agencies and the wider multidisciplinary team. You will maintain a professional portfolio and demonstrate competence across the four pillars of advanced clinical practice.

Job responsibilities
The Trainee Advanced Clinical Practitioner will undertake postgraduate training while consolidating advanced knowledge and skills across the four pillars of advanced clinical practice: clinical practice, leadership, education and research. Working semi-autonomously and within an agreed scope of practice, the post holder will complete comprehensive person-centred assessments, formulate and deliver evidence-based care, monitor outcomes and respond appropriately to deterioration, complexity and risk. They will communicate specialist information clearly, work collaboratively with patients, carers, multidisciplinary colleagues and partner agencies, and maintain accurate clinical records in line with professional, legal and organisational requirements.

The post holder will develop advanced clinical reasoning and decision-making skills to interpret complex, incomplete or conflicting information and make safe, evidence-based judgements. They will prioritise interventions, refer or escalate to appropriate professionals and services, and apply current guidance within their professional scope. The role includes developing knowledge of therapeutics, polypharmacy, optimal prescribing, adverse medication effects and medicines burden, alongside maintaining patient safety and recognising the limits of professional competence.

As part of the leadership and education requirements of the role, the post holder will contribute to clinical leadership, reflective supervision, teaching, mentoring and the development of colleagues and students. They will help identify service and learning needs, support effective use of available resources and maintain constructive relationships across local care systems. In collaboration with the operational manager, clinical assessor and academic assessor, they will maintain a professional portfolio, attend scheduled supervision, complete required assessments and training, and demonstrate progressive competence across all four pillars of advanced clinical practice.

The post holder will also contribute to clinical audit, research, quality improvement, service evaluation and the implementation of robust clinical governance systems. They will critically appraise current evidence and relevant policy, support the development and evaluation of new ways of working, and share learning within and beyond the service. Qualifications
Essential

• Qualified healthcare professional, such as a nurse, paramedic, social worker or occupational therapist.
• Willingness and ability to undertake a PGDip in Advanced Clinical Practice or an equivalent credentialed programme.

Experience
Essential

• Minimum of three years' clinical experience delivering care to a diverse patient population.
• Experience working within healthcare services at an enhanced level of practice.
• Evidence of undertaking comprehensive clinical and risk assessments.
• Understanding of specialist conditions and evidence-based interventions relevant to the service.

Skills
Essential

• Strong clinical reasoning, problem-solving and decision-making skills.
• Effective written and verbal communication skills.
• Ability to build and maintain effective multidisciplinary working relationships.
