Please note the points below;

1. This is a FACE TO FACE role (with no option for remote or hybrid work).
2. This is a FULL TIME role (not part time or flexible hours).

  • Workload & Outcomes Management: Proactively manage GP workload by leading on medicines-related queries and self-care initiatives, focusing on reducing medicine waste and improving long-term patient outcomes.

What You Will Bring
  • Expertise: A deep understanding of medicines optimisation and the ability to apply clinical evidence to real-world, multi-morbid patient scenarios.
  • Collaborative Spirit: The ability to work across multidisciplinary teams (MDTs) to integrate services and provide "joined-up" healthcare.
  • Patient Focus: A commitment to the "shared decision-making" model, empowering patients to take an active role in their own health.
