We’re committed to building a high-quality, sustainable, and patient-centred primary care service for our local communities, and we now have an exciting opportunity for an Advanced Nurse Practitioner / Advanced Clinical Practitioner (ANP/ACP) to join our team.
You’ll be a vital member of our multi-disciplinary Integrated Urgent Care Clinical Assessment Service (IUC CAS) team, based in Segensworth West. Working independently in an Out of Hours setting, you’ll deliver safe, expert clinical care across a variety of formats — telephone triage, face-to-face clinics, and home visits.
You’ll manage the full patient journey independently: taking histories, carrying out physical assessments, forming diagnoses, prescribing treatment, and evaluating outcomes — all within your professional scope and the four pillars of advanced practice (clinical, education, research, and leadership).
Your Role- Independently assess, diagnose, and treat patients of all ages
- Manage acute, non-acute, and chronic conditions
- Prescribe medication within your scope of practice
- Refer patients across care services as needed
- Respond to emergencies and safeguarding concerns
- Ensure safe discharge with clear aftercare plans
- Maintain accurate clinical records per NMC/HCPC standards
What You’ll Need- NMC or HCPC registration
- MSc in Advanced Practice (or equivalent) with Non-Medical Prescribing
- 6+ years post-qualification experience, including 3 years in primary care or urgent care (UTC/ED)
- Level 3 safeguarding certification
- Right to work in the UK
- Enhanced DBS clearance
