Job summaryAre you an Advanced Clinical Practitioner?Primary Care 24 (PC24) are seeking highly skilled and motivated ACPs/ ANPs to join our bank workforce with opportunities within our Community Services based within the Asylum Service.
Main duties of the job- Assess, diagnose, plan, implement and evaluate treatment, interventions and care for people presenting with undifferentiated, undiagnosed, acute, minor, complex or vulnerability-related health needs.
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
- Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition, as appropriate.
- Diagnose and manage both acute and chronic conditions, integrating drug and non-drug-based treatment methods into a management plan.
- Ensure any patient requiring referral for an identified health, social care or safeguarding need is referred or escalated through the appropriate pathway within the required timeframe.

QualificationsEssential- Full NMC Registration with no restrictions
- MSc in Advanced Clinical Practice is desirable; candidate who are working towards this qualification or who have relevant experience are encouraged to apply
- Non-Medical Prescriber V300
- Clinical Examination and Diagnostics Level 7
- Experience of working autonomously
- Experience of working in urgent care and/or primary care
