Salary: £64,876.00 - £66,711.00 per annum (pro rata) plus unsociable enhancements.
Department: Out of Hours. 24/7 service.
Location: Various locations within Merseyside, Sefton & Halton; including Wavertree Head Office.
Shifts: Full time/part time positions available - Weekdays after 6pm and Weekends. Remote working available.
Main duties of the jobAssess, diagnose, plan, implement and evaluate treatment interventions and care for patients presenting with undifferentiated and undiagnosed conditions and complex needs.
Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan.
Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national local protocols and within scope of practice.
Work with patients to support compliance with and adherence to prescribed treatments.
Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions.

What will you be doing- Within your professional boundaries you will assess, diagnose and provide advice, care and management to patients presenting with undiagnosed or undifferentiated illness via telephone consultations, or face to face assessments for patients requiring home visits.
- Undertake face to face and telephone triage consultations
- Interpret pathology/radiology results/reports
- Prescribe and review medication
- Plan, implement and evaluate an effective management plan for the patient
What do you need- You will be an experienced ACP or ANP who is able to deliver quality individualised care to our patients with a high level of autonomy underpinned by the four pillars of Clinical practice, leadership and management, education and research.
- Full NMC Registration with no restrictions
- MSc in Advanced Clinical Practice is desirable; candidates who are working towards this qualification or who have relevant experience are encourage to apply
- Non-Medical Prescriber V300
- Clinical Examination and Diagnostics Level 7
- You will have experience of working in the acute sector, urgent/emergency services environment, or out of hours services at a senior level
