Job Summary
The Prince of Wales Medical Centre has an opportunity for a part-time Practice Nurse working 2 to 4 days a week to join our team.

We are looking for an organised and enthusiastic person with at least 2 years post registration experience and a Post Registration GPN diploma or Practice Nursing Degree or equivalent. The successful candidate will be part of a well-supported clinical team, along with excellent admin and data support staff, serving 8500 + patients in our local area.

Main duties of the job
The Practice Nurse will work with other members of the multi professional team, providing and monitoring the condition and health needs of patients within their care, contributing to the ongoing assessment, as agreed with supervising clinicians and in line with the service pathways. The clinics will cover a range of nursing duties including management of long term conditions (Asthma, Diabetes and COPD), LD and homeless health-checks, Cervical Screening, Sexual Health screening and Contraception, Child Health/Immunisations, Wound Management.

You will take responsibility for Infection Control & Prevention and nursing CQC compliance. Your will oversee clinical stock control and ordering with our Healthcare Assistant.

You will be registered with the NMC and revalidation will be undertaken in line with NMC requirements

You will be working as part of a multi-disciplinary team to monitor care under direct or indirect supervision, using a wide range of skills you have including (but not limited to);

• complement the role of the lead nurse, GP and other Health Professionals
• make the best use of clinical skills within the practice in order to promote well-being and maximise health gain for patients.
• provide a safe, high quality and accessible treatment room service to patients.

As a Practice Nurse you will act within your professional boundaries, providing care for patients from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their care under supervision

You will show safe clinical decision-making and expert care for patients within the general practice, in consultation with the duty GP. You will work with the multi-disciplinary general practice team to support the delivery of policy and procedures and meet the needs of patients.

Supervision and mentoring will be provided by the appropriate senior medical staff - the type of support will depend on the post holders skills and knowledge and determined by our clinical governance arrangements.

Key Responsibilities
The Practice Nurse will work with the nursing team and other members of the multi professional team, providing and monitoring the condition and health needs of patients within their care, contributing to the ongoing assessment, as agreed with supervising clinicians and in line with the service pathways. You will be expected to support patients in the management of long term conditions (Asthma, Diabetes and COPD), Cervical Cytology, Family planning, Child Health/Immunisations, Wound Management, Travel Immunisations, and all other general treatment room duties.

CLINICAL RESPONSIBILITIES

• Deliver high quality, compassionate care with a focus on promoting health, self-care, and independence.
• Carry out specific delegated clinical tasks and responsibilities to a competent standard
• Working without direct supervision (where appropriate) within designated parameters of practice of the Practice Nurse role, accessing clinical and care advice when needed
• Recognise issues relating to safeguarding vulnerable children and adults and report any problems or raise concerns to the appropriate registered care
• Exercise professional accountability and comply with practice policy in relation to the safe administration of medicines
• Use electronic records to record the outcome of assessment and evaluation of care
• Assist in the assessment of and management of risk across the environment where care is being administered
• Assist with the implementation and monitoring of clinical standards and outcomes
• Develop a working knowledge of other providers resources and referral systems to ensure individuals needs are met, within parameters of practice
• Demonstrate good understanding of principles of consent and ensure valid consent is obtained prior to undertaking nursing and care procedures
• Demonstrate a good understanding of the Mental Capacity act / Deprivation of Liberties and applies principles to everyday practice seeking advice / guidance from Registered Nurse or another registered professional as required
• Deliver effective care following plans determined by the registered nurse or other registered care professional and provide feedback on progress against the plan

ADMINISTRATION

• Contributes and participates in audits, evaluation, and clinical standard setting within the Practice
• Accurate and timely summarising of patient records and read-coding patient data
• Complete all required paperwork for legal and administrative purposes in accordance with relevant standards
• Ensure that all practice policies are fully implemented
• Assisting with CQC preparation and compliance.

• Work in accordance with all governance and internal systems relating to the management of clinical data and systems

Experience
Essential

• Understanding of the scope of the role of the Practice Nurse in context of the nursing and interdisciplinary team and the organisation, and how the role may contribute to service development
• Understands and acts in line with NMC professional standards for practice contained within The Code.
• Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice
• Understand requirements for NMC professional Revalidation
• Understanding of the importance of following procedures and treatment plans
• Experience of working in teams under appropriate supervision as part of a multi-disciplinary team
• Insight into how to evaluate own strengths and development needs, seeking advice where appropriate
• Experience of providing and receiving complex, sensitive information
• Ability to take part in reflective practice and clinical supervision activities
• Ability to organise and prioritise own delegated workload

Desirable

• Minimum 2 years experience in general practice
