Job Summary

We have 16 GPs, 4 ANPs, 7 PNs, 6-8 GP Trainees, 3 HCAs and a large, highly performing management team. There are specialisms in GP Training, Diabetes and metabolic medicine, COPD/Asthma, palliative care and womens health.

The pharmacy team comprises of 2 pharmacists, 3 pharmacy technicians and a team of medicine coordinators.

Riverside Health Partnership seeks a part-time/full-time, motivated committed prescribing pharmacist to work with our excellent team of pharmacists, pharmacy techs and medicine coordinators.

We highly value emotional intelligence, good team working skills, flexibility and professionalism apart from clinical skills.

Main duties of the job
There is a focus to develop the team and our medicines management services further with more patient facing clinics and pharmacist-led management of chronic disease, medicines optimisation and managing polypharmacy and appropriate pragmatic prescribing in the elderly amongst other initiatives. Guidance and support for the pharmacy and medicines management team to deal with prescriptions/medicine/prescribing queries is core. The role has a high degree of autonomy.

To work with the GP prescribing lead in the practice and support with achieving efficient and effective prescribing activities.

Working as part of a multi-disciplinary team in a patient-facing role, to provide primary support to general practice staff with regards to prescription and medication queries. Ensuring that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes.

About us
Riverside Health Partnership is a very friendly, supportive and well-run, 4-site training practice of over 24,000 patients based in Bassetlaw. The primary site is Retford, a North Notts market town, by the A1 with other sites at Misterton and Gringley (bordering SE Lincs ans S Yorks) and Harworth (ex mining town off the A1 nr Doncaster).

We are well within the driving distance of Newark, Lincoln, Doncaster, Mansfield, Rotherham and Sheffield.

Only 80mins on mainline to London. Good schooling, affordable housing and an abundance of countryside pursuits.

Job responsibilities
To consult patients, face to face or remotely, to perform effective medication reviews, promote primary prevention and enhance management of chronic diseases under practice protocols and NICE guidance.

Work with other initiatives including safe prescribing in cohorts such as the frail elderly and those with polypharmacy and opportunities for appropriate deprescribing.

To provide guidance to the pharmacy and medicines management team.

Person Specification
Qualifications
Essential
  • Masters degree in pharmacy or equivalent
  • Registered with the General Pharmaceutical Council
  • Qualified prescriber
  • Minimum of 2 years experience as a pharmacist, in acute hospital.
  • Postgraduate clinical qualification and/or significant relevant clinical experience to demonstrate similar level of knowledge
  • Experience of working with the public.
  • Experience of being responsible and accountable for own professional actions, working independently within professional and organisational boundaries and being responsible for the actions of others within their area of responsibility

Desirable
  • Completed Centre for Pharmacy Postgraduate Education (CPPE) Clinical Pharmacist 18-month training pathway.
  • Experience of working within primary care
