Our Prescriptions Management Team works closely with a wide range of clinicians and administrative colleagues, including our PCN Pharmacists and Pharmacy Technicians who are co-located within Ossett Surgery. Main duties of the jobThe Pharmacy Technician will play a key role in supporting safe, effective and efficient medicines management for a patient population of approximately 22,000 at Ossett Surgery.
Working as part of our established Prescriptions Management Team and alongside our co-located PCN Pharmacists and Pharmacy Technicians, the postholder will contribute to high quality patient care through the processing of acute medication requests, undertaking basic medication reviews within their competency, and ensuring robust medicines optimisation processes across the practice.
The role will support the practice by managing DOAC reviews, delivering cost effective prescribing initiatives, actioning medication related tasks and queries, and responding to MHRA safety alerts. The Pharmacy Technician will complete medicines reconciliation following hospital discharges and clinic letters, identify and escalate prescribing discrepancies, and implement local and national prescribing priorities. They will also contribute to quality improvement through audits, monitoring of high risk medicines, and completion of CQC related safety searches.

Role Specific ResponsibilitiesA key part of the role is to strengthen, and streamline prescribing processes by supporting, training and developing administrative colleagues. This includes improving coordination, accuracy and efficiency within pharmacy related administration, particularly around medication requests, prescription queries and repeat prescribing workflows.
This position is pivotal to improving the quality of care and operational efficiency within Ossett Surgery. It requires motivation, initiative and a commitment to delivering excellent service within general practice. As our services continue to evolve, the postholder will be expected to adapt and undertake the duties outlined in the job description to support safe, effective and patient focused prescribing.
Specific tasks include (This list is indicative, not exhaustive):- Process acute medications requests.
- Medicines optimisation and assisting with medication queries from patients, care homes and community pharmacies and escalate as appropriate.
- Conduct medicine reconciliation for discharge letters, outpatient letters and new patient registrations
- Complete any follow up actions from discharge and outpatient letters.
- Identify and resolve medication discrepancies and liaise with Pharmacists and GP's
- Promote medicines synchronisation and patient adherence.
- Support the implementation of local and national cost saving prescribing initiatives including performing medication switches as directed by prescribers, pharmacists and the medicines optimisation team.
- Maintain MHRA alerts and high-risk drug monitoring records
- Support safe prescribing and monitor high-risk medications
- Monitor CQC safety searches and escalate when required.
- Support the recall of patients (where needed)
- Contribute to medicines safety alerts implementation
- Lead and contribute to audits and quality improvement initiatives relating to the effective prescribing of medicines
- Analyse prescribing data and share insights with the wider team
- Support patient education and promote safe care and patient self-management
- Complete medication reviews (up to 5 items - dependent on competence)
- Complete DOAC reviews
- Complete follow up actions following prescribing related patient safety events.
Person SpecificationExperienceEssential- Experience working as a qualified registered Pharmacy Technician in Primary Care, PCN, Community Pharmacy or hospital setting
- A good understanding of Primary Care Services and the NHS structure
- Knowledge of GDPR, confidentiality and safeguarding
- Attention to detail and an organised approach to work
- Excellent oral and written communication skills
- Able to work as part of a team and autonomously
- Understanding of Pharmacy procedures
- Understanding of medicines legislation and ethics
- IT Skills including MS word, Outlook & Excel
- Experience in a patient facing role
Desirable- Experience of working in a General Practice setting
- Experience using the SystmOne clinical system
- Experience supporting audits and quality improvement initiatives
- Experience dealing with patient queries and providing advice within scope of practice

QualificationsEssential- Pharmacy Technician qualification
- Evidence of CPD
- GCSE or equivalent in Maths & English (grade C/4 and above)
- Committed to CPD and the completion of the CPPE PCPEP pathway (if not already completed)
Desirable- BTEC level 4 Diploma in Clinical Pharmacy Services or a certificate of Higher Education in Pharmacy Technician Practice
