Job summaryThe role will provide specialist assessment, diagnosis, treatment and advice to patients and their carers. The caseload will include many acute and chronic care cases with complex disabilities and needs. Some services will also include terminal care cases.
You will also be part of a Multidisciplinary team working in a community setting, including Urgent Care Response.
You will ensure that therapy input is integrated into the patients care plan.
Work as an autonomous practitioner working without direct supervision and, sometimes, lone working.
Provide supervision, training and support to junior staff and students.
Work with managers to develop the service in line with patient need and trust-wide developments and to help provide an equitable service across the Trust.
Some services are currently required to participate in on-call and weekend work.

In South Molton Hospital we are looking to recruit dynamic and highly motivated Physiotherapist to join our Community Inpatient Rehabilitation Team.
You will be based on Hugh Squire Ward at South Molton Community Hospital, located in the heart of the market town of South Molton on the edge of the beautiful North Devon countryside. Our catchment area includes the surrounding rural communities across North Devon, offering the opportunity to work within a close-knit and supportive community. South Molton enjoys excellent connections to both North Devon and Exeter, with convenient access to the North Devon Link Road (A361), providing links to the M5 motorway, Exeter and beyond. The area offers an excellent quality of life, combining rural charm with easy access to stunning countryside, coastlines and local amenities.
We are committed to developing and supporting our staff in South Molton, striving to create a happy and healthy working environment. We seek a forward-thinking, team-orientated Physiotherapist to join our small but friendly and supportive team. You will work closely with other Physiotherapists, Occupational Therapists, and Rehabilitation Support workers to support patients presenting with various acute and long-term conditions in specialities, including Musculoskeletal, Respiratory, Trauma, Geriatrics, Vascular and Falls.
You will be responsible for high-quality assessment, treatment planning, and management of rehabilitation programs for a wide variety of clinical presentations. You will manage your caseload, including appropriate delegation, and supervise and support junior team members.
The post holder must have relevant clinical experience. You must be able to demonstrate flexibility to support the needs of the service as required. Prior community experience is desirable but not essential.
