Job summary
Royal Surrey NHS Foundation Trust delivers acute and urgent care services within the community, supporting patients who would otherwise require hospital admission. The service provides rapid assessment, diagnostics, and treatment for individuals with complex health and social care needs, frailty, or exacerbations of long-term conditions.

You will be part of a multidisciplinary team providing assessments and interventions within patients' own homes, residential care homes, and nursing homes to prevent further deterioration in their health and wellbeing. Early identification, assessment, and intervention are crucial in reducing avoidable hospital admissions and supporting patients to remain safely within their community setting.

The service aims to deliver urgent case management, crisis response, recovery support, and self-management strategies, while adopting a collaborative and patient-centred approach to care. You will work closely with patients, their families and carers, GPs, and other healthcare professionals to ensure coordinated and effective care delivery.

The operational hours for this service are 8:00am to 8:00pm, 7 days a week.

Main duties of the job
You will be part of a fast-paced and continually evolving Community Multidisciplinary Team (MDT), where your ability to make high-level clinical decisions will play a key role in supporting patient recovery and preventing unnecessary hospital admissions. This will involve working closely with patients' GPs, community services, and other healthcare professionals across primary and secondary care.

You will provide clinical leadership within the Urgent Community Response (UCR) and Hospital at Home teams, delivering high-quality, patient-centred care to support individuals with acute and complex health needs in their own homes. The aim of the service is to prevent avoidable hospital admissions, facilitate early supported discharge, and promote recovery within the community.

The role includes responsibility for the day-to-day coordination of service delivery, including the management and prioritisation of patient caseloads, ensuring timely assessments and interventions, and supporting effective multidisciplinary working. You will supervise, support, and develop team members, ensuring appropriate staffing levels and skill mix to maintain safe and effective care.

You will be expected to work autonomously, undertaking comprehensive clinical assessments, risk assessments, and complex decision-making, while seeking support and guidance from colleagues and specialist services when required.

Qualifications/Professional Registration
Essential

• NMC registered qualification
• Practice supervisor/Practice Assessor
• Evidence of a range of clinical competencies appropriate for the role.
• Degree level qualification in relevant training or equivalent knowledge and experience in health related subject

Desirable

• Relevant urgent care, community, frailty or long term conditions related training
• Health Assessment course
• Non -Medical Prescriber

Knowledge and Experience

• Experience of delivering care in acute, community, intermediate care or urgent care setting.
• Experience of working independently whilst recognising professional boundaries
• Evidence of CPD aligned to the role
• Understanding of integrated care systems and community pathways
• Leadership and supervision skills

Desirable

• Experience in urgent community response, virtual ward, frailty or admission avoidance services
• Knowledge of the national UCR services specifications (2 hour response model)
