Job summaryOlder Adult Home Treatment Team (OAHTT) Hours: 37.5 per weekSalary: Band 5 £32073 - £39043 pa (dependant on experience)Location: SHSC 7 Edmund Road, Sheffield, S2We are looking for an enthusiastic and motivated registered mental health nurse to join our Older Adult Home Treatment Team.
The Older Adult Home Treatment Team provides a service 8-8pm, 365 days a year to older adults experiencing a crisis in the context of functional mental health or dementia. We work intensively with service users in a community setting to prevent admission to a mental health ward, support discharge from a mental health ward and provide comprehensive assessment and treatment plans in the person's own home.
You should be able to demonstrate your passion towards working with this client group and have the ability to assess, co-ordinate treatment and support clients and carers. You should possess effective communication skills and have the ability to work under your own initiative and as part of a wider Multidisciplinary team.
Main duties of the jobThe post holder will have continuing responsibility for the provision of skilled intervention of people with assessed and diagnosed as having significant mental health problems in the community.
The post holder will work within a multi-disciplinary service under the supervision of experienced staff, and core responsibilities will include:
- Acting as a Care Co-ordinator/Lead Professional.
- Work within Care Programme Approach.
- Carrying a varied caseload defined by need.
- Contribute to a duty system on a rota basis.
- Contribute to multi-disciplinary service development.
The post holder will:- Value and be responsive to the individual rights, needs and opinions of the people using our services, in the performance of all the activities of the Community Mental Health Nurse
- Be responsible for the ongoing assessment, planning, implementation and evaluation of programmes of nursing care to an agreed standard.
- Undertake initial nursing assessments, care planning and risk assessments as agreed.
- Participate in monitoring the standard of care delivery by Support Worker staff and the education of colleagues, learners.
- Maintain and promote a patient centred approach in all aspects of service delivery to people and their carers

Job responsibilitiesClinical- To work across a variety of community settings as part of a multi-disciplinary team.
- Providing a nursing assessment and identification of a referred clients mental health needs at home/in the persons own environment, in circumstances when they have been seen by another member of the multi-disciplinary team.
- Formulating, implementing and evaluating collaborative care plans that reflect the needs of the clients/carers over a 24-hour period.
- Manage own caseload with a degree of autonomy having responsibility for the nursing care of clients until point of discharge. Regularly review and discuss caseload management with their clinical supervisor
- Administering depot injections to clients and monitoring the effects of prescribed medication.
- Maintain accurate care records about care interventions and treatment offered according to Professional and Trust standards.
- To act as co-ordinator in accordance with the Care Programme Approach.
- Provide comprehensive written mental health specialist nursing reports as required.
- Delivering a range of therapeutic interventions with clients and carers in accordance with evidence based practice and Trust Policies
- Reacting to and managing a range of complex mental and physical health crises.
- Escorting clients where appropriate to other Health and Social Care settings.
- Offering support and advice to families and carers and providing advocacy to clients and carers where appropriate
- Identify and manage risks in a variety of as well as document them in DRAMs and risk reduction plans.
Supervision- Managing and maintaining a caseload with appropriate levels of supervision
- To provide direct supervision to junior staff and contribute and assist with PDR system.
- Liaising with, advising and educating a range of other professionals, agencies and bodies involved in the care of older people with mental health needs.
- To act as identified supervisor/mentor for nursing students on allocated placements.
- Receive clinical supervision from agreed supervisor and manager on a regular, planned basis to ensure effective and safe management.
Service development- To participate with colleagues in the production of an active learning environment in order to maintain and develop high standards of professional and clinical practice. Promoting opportunities for self, colleagues and junior staff to access available training / education.
- Sharing identified skills with colleagues, learners and junior staff, participating in formal and informal teaching as appropriate.
- To take responsibility for identifying personal development needs, participating in appraisal and clinical supervision

- Offering education and health promotion to clients and carers

Person SpecificationExperienceEssential- Experience of working with Older Adults in a mental health setting
- Experience of assessment and care planning
- Experience of multi disciplinary working
Desirable- Relevant post qualifying experience in mental health nursing
- Experience of working with older adults in a community setting
TrainingEssential- Registered Mental Health Nurse qualification.
- Evidence of current or pending post qualification NMC registration.
Knowledge and skillsEssential- Good risk assessment and risk management skills including the use of assessment tools.
- Knowledge and understanding of the mental health needs of older people.
- Goal planning and service user engagement skills
- Demonstration of commitment to on-going personal & professional development
