Job summary
We are looking to recruit an enthusiastic, motivated and experienced Older Adult RMN to join our well-established Liaison Psychiatry team. Our team provides a 24 hour service to the Sheffield Teaching Hospitals (STH). We are looking to recruit a specialist nurse to work with older adults, 365 days a year during core hours (8am-6pm) to support the function of the team. The specialist nurse will work across all in-patient areas of STH (including the emergency department) to provide specialist assessment, risk assessment and management plans including discharge plans.

The specialist older adult nurse will provide comprehensive mental health assessment, formulation, risk management and short-term intervention for older adults presenting within the acute hospital setting. The post holder will work autonomously and collaboratively within a multidisciplinary liaison psychiatry team to triage and assess older adults who are referred to the Liaison Psychiatry team. The role will include liaison with other agencies aswell as supervision and teaching of other staff and students.

Main duties of the job
  • Able to work independently in a number of environments as an autonomous practitioner.
  • Provide comprehensive specialist mental health assessment of a referred patients' mental health needs, formulating and implementing nursing care plans and goals which will reflect the needs of the patients over a 24-hour period.
  • Regularly review caseload management with their clinical supervisor.
  • Maintain accurate records about the care and treatment offered.
  • Ensure that clinical, supportive, and therapeutic interventions protect the rights of the patient and are in accordance with evidence-based practice and Trust policies.
  • Adhere to confidentiality principles and maintain the rights of individual patients.
  • Contribute personal and professional skills to ensure the development of the Mental Health Team
  • Identify and manage risks in variety of settings.
  • Offering support and advice to families and carers and acting as an advocate for clients, where appropriate.
  • Contribute to managing the team caseload by the effective use of time and resources.
  • Participate in regular supervisions, individual appraisal performance reviews and the construction of personal developments plan.

Job responsibilities
The Liaison Psychiatry Service is a multi-disciplinary and innovative team, providing triage and assessment of a wide range of service users. You will be expected to work to accreditation standards as part of your ongoing practice.

The specialist older adult mental health nurse will be expected to work across 7 days during core hours (8am-6pm), including Bank Holidays.

The specialist nurse will need to conduct robust triage and comprehensive biopsychosocial assessment of older adults who present with a wide range of needs including delirium, cognitive impairment and those presenting in mental health crisis.

The specialist nurse will have knowledge of working with individuals who experience ideas of suicide, self-harm, acute mental illness presentations and dementia / confusion / delirium. Main responsibilities include:
  • Undertake comprehensive biopsychosocial and mental health assessments within Sheffield Teaching Hospitals emergency department and in-patient areas.
  • Assess and manage risk relating to suicide, self-harm, vulnerability, aggression, neglect and safeguarding concerns.
  • Develop and implement collaborative care and safety plans.
  • Provide specialist mental health advice, consultation and support to acute hospital staff.
  • Facilitate timely referrals and discharge planning with community and inpatient services.
  • Work effectively with older adults presenting with severe mental illness, dementia, delirium, neurodevelopmental conditions and/or substance misuse.
  • Support assessment and management under the Mental Health Act and Mental Capacity Act.
  • Promote trauma-informed, recovery-focused and least restrictive practice.
  • Deliver brief therapeutic interventions and crisis management where appropriate.
  • Maintain accurate, timely and legally compliant clinical records.
  • Participate in clinical supervision, reflective practice and continuing professional development.
  • Contribute to teaching and training of colleagues and partner agencies.
  • Support quality improvement initiatives, audits and service evaluation.

  • Develop effective working relationships with service users, carers and multidisciplinary teams.
  • Communicate sensitive information clearly and compassionately.
  • Provide professional advice to acute hospital clinicians and external agencies.
  • Liaise with community teams, crisis services, social care, police, ambulance services and voluntary organisations, as required.

  • Contribute to service improvement and implementation of best practice guidance.

Qualifications
Essential
  • Registered Mental Health Nurse (RMN) with current NMC registration
Experience
Essential
  • Experience of working in an MDT setting as a Band 5 Mental Health Nurse
  • Experience of working with older people in an acute and/or community setting
  • Experience of crisis assessment, risk management and multidisciplinary working
  • Experiencing of supervising junior staff and students
  • Commitment to recovery-focused, trauma-informed and person-centred care
  • Ability to travel independently across service locations where necessary
  • Awareness and understanding of relevant Mental Health legislation
Qualifications
