Job summaryOur PCN is seeking a part time Frailty Nurse (band 7 equivalent) to join our established Frailty Team, working 1 day a week. The applicant will join a second part time Frailty Nurse to provide clinical oversight and specialist nursing care for patients with moderate and severe frailty, working collaboratively with Care Coordinators, GPs, pharmacists, community services and the wider multidisciplinary team within the PCN.
Main duties of the jobThe role focuses on acute care, as well as proactive frailty management. The post holder will complete Comprehensive Geriatric Assessments, and support the Frailty Team in anticipatory care planning, prevention of avoidable hospital admissions, and delivery of person-centred care for older people with moderate to severe frailty, helping them remain to independent and avoid unnecessary hospital admissions.
The role will support identification of undiagnosed barn door dementia and, following GP input, present these at the Dementia Co-Diagnosis MDT.
The post holder will work autonomously within their scope of practice, undertaking advanced clinical assessments, diagnosis, treatment and prescribing where appropriate.

Job responsibilities- Provide holistic assessment including physical, psychological, functional and social needs
- Undertake Comprehensive Geriatric Assessments (CGA)
- Independently prescribe medication where qualified and appropriate
- Develop personalised care and support plans
- Support anticipatory care planning and advance care planning discussions
- Undertake home visits for housebound and vulnerable patients
