We are seeking an enthusiastic and compassionate Diabetes Specialist Nurse who is passionate about delivering high-quality, patient-centred care and empowering people to manage their condition effectively.

Applications will be considered from NMC registered Nurses currently working within Diabetes who have relevant experience and are in the process of completing the Non medical prescribing course. For candidates who have not yet completed this, the post will be offered as a developmental post at the appropriate Annex 21 salary in accordance with Agenda for Change terms and conditions.

Main duties of the job
Working as an autonomous practitioner within a supportive multidisciplinary team, you will collaborate with consultants, specialist nurses, primary care colleagues and other healthcare professionals to deliver high-quality, evidence-based diabetes care. You will play a key role in shaping and developing services through quality improvement, audit, research and education, helping to improve outcomes for people living with diabetes across our local community.

This role is ideal for an enthusiastic and experienced nurse with specialist diabetes knowledge, strong leadership and communication skills, and a commitment to delivering outstanding patient-centred care.

Key Responsibilities

• Provide specialist assessment, treatment and ongoing management for patients with complex diabetes.
• Lead nurse-led clinics and support the delivery of inpatient diabetes services.
• Triage referrals and develop appropriate management plans.
• Deliver education and training to patients, carers and healthcare professionals.
• Act as a diabetes expert, offering specialist advice and support across the Trust.
• Contribute to service development, audit, research and the implementation of best practice.
• Support the learning and development of nursing colleagues and students.

• Non medical prescribing (or willingness to undertake)

• Minimum 3 years post registration Multidisciplinary team working 11 at band 6 with experience in working with patients with Diabetes.
