Virtual Care Practitioner

• Band 7 NHS Agenda for Change - £49,387 to £56,515 per annum

At South East Coast Ambulance Service NHS Foundation Trust (SECAmb), our Virtual Care service is at the forefront of clinical transformation. We are seeking experienced clinicians who are passionate about patient care and ready to play a key role in shaping the future of urgent and emergency healthcare.

As a Virtual Care Practitioner, you will manage complex, high-risk and undifferentiated patient presentations, providing expert clinical decision-making and senior support within our Virtual Care service. Alongside delivering exceptional patient care, you will provide leadership, mentorship and clinical guidance to colleagues, whilst contributing to service development, innovation and quality improvement.

Main duties of the job
As a Virtual Care Practitioner, you will:

• Undertake enhanced virtual assessment and management of patients accessing 999, NHS 111 and other referral pathways.
• Manage complex and high-risk patients using advanced clinical reasoning, evidence-based practice and professional judgement.
• Formulate differential diagnoses and develop safe, effective and patient-centred management plans.
• Provide expert clinical advice, leadership and support to colleagues across the Virtual Care service.
• Lead, coach and mentor clinicians, supporting professional development and workforce capability.
• Support the development and optimisation of referral pathways and innovative models of care.
• Contribute to quality improvement, clinical governance, education and service transformation initiatives.
• Work collaboratively with multidisciplinary teams and healthcare partners to improve patient outcomes across the healthcare system.

Alongside delivering expert clinical care, you will play a key role in the development of colleagues, providing clinical leadership, mentorship and support across the service. You will work collaboratively with multidisciplinary teams and healthcare partners, helping to shape clinical pathways, improve patient outcomes and influence how Virtual Care develops across the Trust.

Person Specification
Qualifications
Essential

• Registered Paramedic (HCPC) or Registered Nurse -Adult (NMC) with current professional registration.
• Evidence of relevant Level 7 study, postgraduate education or formally evidenced portfolio based development that supports clinical assessment, differential diagnosis and management of patients, or equivalent qualifications.
• Specialist Practice qualification in Urgent, Emergency, Primary Care or equivalent. First Contact Practitioner (FCP) training or portfolio. Non-Medical Prescribing qualification. Supervision, mentoring or clinical education qualification.

Knowledge & Experience
Essential

• Enhanced knowledge of clinical assessment, differential diagnosis, clinical decision making and risk management. Knowledge of clinical supervision, professional development and workforce development approaches.
• Significant experience of autonomous clinical assessment, decision making and management of patients within urgent, emergency, community or unscheduled care settings.
• Evidence of commitment to continuous personal and professional development, reflective practice and lifelong learning.
