Job summary
We have an exciting opportunity to be part of the Hospital at Home and Urgent Community Response Team Tees. The ACP role is embedded in the medical workforce in Hospital at Home and you would be joining a team of ACPs, Nurse Practitioners and General Practitioners to provide high quality, safe and effective care to patients.

We are looking to recruit a qualified ACP with experience in acute medicine/ older persons medicine/Hospital at Home services. Candidates must have completed the ACP MSc programme and have a completed portfolio of professional development which evidences an advanced level of clinical practice in line with HEE (2017) framework for ACP along with evidence of ongoing professional development.

Main duties of the job
This role will be at an advanced clinical level in the direct delivery of high-quality care making critical clinical decisions based on advanced clinical skills assessment . Working effectively across all four pillars of advanced clinical practice as set out by Health Education England (2017).

The post holder will be able to evidence all four pillars of the advanced clinical practice framework and benchmark to advanced clinical practice levels across all four pillars.

The post holder will provide clinical leadership and support within the nursing/AHP teams in developing the service to improve the patient experience.

You will be/have:

• Registered healthcare practitioner
• MSc in advanced Clinical Practice/Royal college credentialed/ HEE accredited ACP
• Completed portfolio of professional development which evidences an advanced level of clinical practice in line with HEE (2017) framework for ACP
• Independent non-medical prescribing
• Portfolio demonstrating continued professional development and ongoing assessments
• ILS/ALS provider

Experience
Essential

• Extensive clinical experience in speciality
• Has led audit and research within the service.
• Can manage a highly complex caseload.
• Can refer independently to other members of the MDT.
• Has led on the development of standard operating procedures (SOPs) and/or local policy.
• Works within a supervision framework for ACP
• Supports Junior doctors/nurses/AHPs to develop their own scope of practice.
• Experience leading a team.

Desirable

• Experience in one of the four pillars of ACP at an extended level

Qualifications
Essential

• MSc in advanced Clinical Practice/Royal college credentialed/ HEE accredited ACP (Band 8a)
• Completed portfolio of professional development which evidences an advanced level of clinical practice in line with HEE (2017) framework for ACP (Band 8a)
• Independent non-medical prescribing (where legally permitted) (Band 8a)
• Portfolio demonstrating continued professional development and ongoing assessments
• ILS/ALS provider
• Formal research module completed at level 7 (master's level).
• Will work towards and complete any appropriate credential routes which are available and applicable to the clinical area.
• Registered healthcare practitioner

Knowledge
Essential

• Professional knowledge acquired through a professional qualification which enables registration with a health care regulating body.
• Supplemented by significant post-registration experience and specialist knowledge of the clinical area.
• Management skills gained through theory and / or significant practice experience
• Leadership skills
• Ability to autonomously problem solve, analyse, compare, and interpret complex information before making a decision on patient care.
• Has physical skills to ensure accuracy and dexterity required for clinical procedures.
• Has the ability to travel independently between patients own homes.
