We have an exciting opportunity for an Advanced Nurse Practitioner / (ANP) or Advanced Clinical Practitioner (ACP) to join our team. We are looking for a motivated and experienced clinician - Nurse Practitioner or Paramedic with a background in Primary Care or urgent care, who is confident working autonomously and managing patients independently. We would be particularly interested inexperience or interest in home visiting.

This role offers up to 37.5 hours per week, part time applications welcome with additional sessions available through our extended hours service. We have opportunities for ongoing training, professional development, and career progression. Facilitated peer-to-peer supervision sessions.

Main duties of the job
You will provide expert autonomous clinical care to patients. Working at an advanced level of practice, you will undertake assessment, diagnosis, treatment, and management of patients. The role operates across the four pillars of advanced practice: clinical practice, leadership/management, education, and research, contributing to service development, workforce transformation, and improved patient outcomes.

St Augustine's Medical Practice is based in the new Chocolate Quarter development in Keynsham, we have a branch surgery at Temple House in Keynsham Health Centre and in nearby Saltford.

Our purpose built modern spacious premises, easily commutable from Bath/Bristol and surrounding areas and a short walk from Keynsham train station / high street. There are many amenities at the Chocolate Qtr including an on-site gym/spa facility, with pool, offered at a discounted rate for NHS staff, as well as coffee shop and restaurant.

Our friendly nursing team of ANP's, Practice Nurses, Nursing Associates and HCAs is part of our diverse and changing workforce, which consists of GPs, FCP's, paramedics, pharmacists, and other allied health professionals.

We have regular meetings to manage and continually improve our patient care. We are passionate about training the next generation of workforce having been teaching practice for many years.

We are part of the Keynsham Primary Care Network (PCN) which includes West View. Working together allows us to take on bigger projects that we couldn't do alone, share good practice and improve care for patients.

Job responsibilities

• Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with undifferentiated and undiagnosed conditions and complex needs.
• Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.
• Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
• Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan.
• Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national/local protocols and within scope of practice.
• Work with patients in order to support compliance with and adherence to prescribed treatments.
• Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
• Prioritise health problems and intervene appropriately to assist patients in complex, urgent or emergency situations, including initiation of effective emergency care.
• Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care, contributing to making every contact count.

Qualifications
Essential

• Advanced Clinical Practitioner
• MSc in Advanced Clinical Practice (or working towards)
• Non-Medical Prescriber V300
• Clinical Examination and Diagnostics Level 7
• Experience of working in paediatrics
• Experience of working in urgent care and/or primary care
• Long term conditions qualification
• Mentor/teaching qualification
• Trained clinical supervisor
• Full UK driving licence

Desirable

• Relevant post graduate diploma or degree
• Non-medical prescriber qualification
• Emergency Care Practitioner course or similar
• Aware of and/or working towards First Contact Practitioner Roadmap for Paramedics (Paramedic only)

Experience
Essential

• At least 3 years experience
• Experience in the acute examination of adults and children
• Management of common minor injuries/illnesses
• Experience as an autonomous practitioner
• Able to work under pressure and manage varying workloads and changing demands
• Evidence of ongoing post registration CPD
• An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing

Desirable

• Experience of working in Primary Care / Urgent Care / Emergency Care
• Broad knowledge of general practice and the new NHS landscape including relationship between individual practices / Primary Care Networks (PCN)
• Experience of visiting patients in their own homes
