Are you a motivated Pharmacy Technician looking to develop your clinical role in Primary Care?

We have an exciting opportunity for a Pharmacy Technician to join our established team, delivering a new practice-based Hypertension and Lipid Management Service across our surgery and Primary Care Network.

Working as part of a forward-thinking multidisciplinary team, you will provide structured patient reviews, support home blood pressure monitoring, promote medicines optimisation, deliver lifestyle interventions and help patients reduce their cardiovascular risk. You will work closely with our experienced Clinical Pharmacists, who provide daily supervision, clinical support and prescribing expertise, ensuring you are fully supported in your role and professional development.

Main duties of the job
What you'll be doing

• Conducting telephone & face-to-face patient reviews.
• Supporting patients with hypertension & cholesterol management.
• Assessing medicines adherence & identifying barriers to treatment.
• Providing education on lifestyle changes & cardiovascular risk reduction.
• Reviewing home blood pressure readings & monitoring results.
• Coordinating blood tests & follow-up monitoring.
• Documenting consultations within SystmOne.
• Referring patients to the Clinical Pharmacist team for medication review & prescribing decisions.
• Supporting the achievement of QOF
• Supporting medicines reconciliation activities

About You
We are looking for someone who:

• Is a registered Pharmacy Technician with the GPhC.
• Has excellent communication & patient engagement skills.
• Enjoys working as part of a collaborative MDT.
• Has an interest in cardiovascular disease prevention & medicines optimisation.
• Is organised, proactive & committed to delivering high-quality patient care.
• Previous Primary Care experience is desirable but not essential.

Job responsibilities
Job Purpose
The Pharmacy Technician will work as part of a multidisciplinary Primary Care Network team to to improve the quality, safety and and effectiveness of medicines optimisation and prescribing processes.

Working under the supervision of the Clinical Pharmacists and within the scope of professional practice, the post holder will support medication reviews, medicines reconciliation, repeat prescribing systems, medicines safety initiatives, prescribing audits, care home medicines management and patient support activities. The role aims to enhance patient outcomes, reduce medicines-related harm and improve the efficiency of general practice services.

Key Responsibilities
Medicines Optimisation

• Support the delivery of medicines optimisation services across the PCN.
• Undertake medication reviews within agreed protocols and competence.
• Identify patients requiring medication review and assist in managing prescribing priorities.
• Promote cost-effective prescribing in line with local and national guidelines.
• Support deprescribing initiatives and structured medication review programmes.

Medicines Reconciliation

• Reconcile medicines following discharge from hospital, outpatient appointments and other care settings.
• Ensure timely updating of clinical records.
• Resolve medication discrepancies through liaison with healthcare professionals.
• Escalate clinical concerns appropriately to the supervising Clinical Pharmacist or GP.

Repeat Prescribing Management

• Manage and improve repeat prescribing systems.
• Process prescription requests within agreed protocols
• Identify and address issues relating to medication synchronisation and prescription ordering.
• Support patients to use repeat prescribing services effectively.

Medicines Safety

• Support implementation of national medicines safety guidance, MHRA alerts and local safety initiatives.
• Participate in prescribing safety audits and quality improvement programmes.
• Assist with monitoring of high-risk medicines and medicines requiring regular review.
• Contribute to reducing prescribing errors and medication-related incidents.

Care Home Support

• Support medicines management in care homes served by the PCN.
• Assist with medication audits, stock reviews and medicines-related queries.
• Work collaboratively with care home staff to improve medicines safety and patient outcomes.

Patient Care

• Provide advice and support to patients regarding medicines use and adherence within competence.
• Respond to routine medication queries from patients and carers.
• Promote self-care and shared decision-making regarding medicines.
• Signpost patients appropriately when issues fall outside own scope of practice.

Clinical Systems and Data Quality

• Ensure accurate documentation of medicines-related interventions.
• Maintain high standards of clinical coding and record keeping.
• Use GP clinical systems to identify patients for review and support population health initiatives.
• Contribute to data collection and reporting requirements.

Multidisciplinary Team Working

• Work collaboratively with GPs, Clinical Pharmacists, nurses, social prescribers and wider healthcare professionals.
• Participate in multidisciplinary team meetings where appropriate.
• Support the development of integrated medicines optimisation services across the PCN.

Professional Responsibilities

• Maintain registration with the General Pharmaceutical Council (GPhC)
• Work within the standards of conduct, ethics and performance for pharmacy professionals.
• Maintain professional competence through continuing professional development.
• Participate in appraisal, training and supervision arrangements
• Adhere to all NHS, PCN and practice policies including safeguarding, information governance & confidentiality

Person Specification
Experience
Essential

• Experience working as a Pharmacy Technician within community pharmacy, hospital pharmacy, primary care or another healthcare setting.
• Experience of medicines management processes.
• Experience using pharmacy and/or clinical IT systems.
• Understanding of medicines optimisation principles.

Qualifications
Essential

• Registered Pharmacy Technician with the General Pharmaceutical Council (GPhC).
• BTEC/NVQ Level 3 in Pharmaceutical Sciences or equivalent recognised qualification.
• Evidence of continuing professional development.

Knowledge & Skills
Essential

• Understanding of NHS primary care and general practice.
• Knowledge of repeat prescribing processes.
• Understanding of medicines reconciliation.
• Awareness of medicines safety and governance requirements.
• Understanding of confidentiality, data protection and safeguarding.
