Job summaryAn exciting opportunity has arisen for an experienced and motivated pharmacist to join the Swansea Bay University Health Board Pharmacy Team as a Band 8a Specialist Pharmacist supporting inpatient cancer services at Singleton Hospital.
The successful post holder will deliver clinical pharmacy services to cancer inpatients, providing expert pharmaceutical care for patients receiving systemic anti-cancer therapy (SACT), and other complex treatments. Working as part of the multidisciplinary team, the post holder will undertake medicines reconciliation, clinical review, medicines optimisation, discharge planning, specialist medicines information and support the safe delivery of inpatient cancer treatments.
The Cancer Pharmacy Team is a highly specialised, supportive and innovative service dedicated to delivering safe, effective and patient-centred care. The post holder will also contribute to service development, workforce education, clinical governance and quality improvement.
Applicants must be registered with the GPhC and have substantial post-registration clinical experience. Previous oncology or haematology experience is desirable but not essential, and applications are welcomed from pharmacists with strong clinical skills wishing to develop a specialist career in cancer pharmacy. A comprehensive induction and ongoing development programme will be provided.
Successful candidates will be appointed to the role of Advanced Pharmacist (Band 8a).
Main duties of the jobAs a Specialist Cancer Pharmacist, you will play a key role in delivering and developing a high-quality clinical pharmacy service to inpatient haematology and oncology wards. Working as an integral member of the multidisciplinary team, you will provide expert pharmaceutical care to patients with complex conditions, supporting the safe and effective use of high-risk medicines, optimising treatment, and contributing to improved patient outcomes throughout their inpatient stay.
The role will involve undertaking comprehensive medicines optimisation, clinical review, medicines reconciliation, therapeutic monitoring, prescribing where appropriate, and supporting timely discharge planning, including the management of complex discharge prescriptions. You will work closely with medical, nursing and allied health professional colleagues to ensure the highest standards of patient care for individuals receiving systemic anti-cancer therapies and supportive treatments.

The post holder will provide specialist clinical pharmacy services to cancer inpatients at Singleton Hospital. Working within the multidisciplinary team, they will support the safe, effective and timely use of medicines throughout the cancer pathway and contribute to the development of cancer pharmacy services.
Key responsibilities include delivering specialist clinical pharmacy care; providing expert advice on oncology, haematology and supportive care medicines; supporting the safe prescribing, verification and governance of systemic anti-cancer therapy (SACT); participating in ward rounds, MDTs and discharge planning; developing policies, guidelines and shared care protocols; investigating medication incidents; undertaking audit, research and quality improvement; providing education and clinical leadership to pharmacy and multidisciplinary colleagues; supporting service development and innovative models of care; and maintaining professional competence through continuing professional development.
Previous experience in cancer services is desirable but not essential. The successful candidate will receive structured training, including specialist oncology education, supervised practice and completion of SACT competencies where required.
You will also contribute to the ongoing development of the cancer pharmacy service through participation in clinical audit, service evaluation, quality improvement initiatives, education and training, medicines governance, and the monitoring and analysis of medicines expenditure, helping to ensure safe, effective and efficient use of medicines across the Directorate.
Person SpecificationQualifications & KnowledgeEssential- Post Graduate MSc/ Diploma in Clinical Pharmacy or equivalent experience.
- Member of the GPhC.
- Independent Prescriber (or complete within agreed timeframe upon employment).
ExperienceEssential- Experience as clinical pharmacist in specialist area.
- Experience of contributing into the pharmaceutical care in specialist area.
- Ability to work in a highly specialist area.
Desirable- Proven track record of competence. Evidence of clinical audit, practise research, implementation of procedures, protocols.
Aptitude & AbilitiesEssential- Innovative and well organised.
- Good communicator, motivated, able to work as part of a team.
- Ability to concentrate when frequently under pressure and subject to interruptions.
