An exciting opportunity has arisen for a full time post for Community Mental Health Nurse to join our Community Mental Health Team, based at Sovereign House Hartlepool older peoples service. We are looking for a dynamic, positive and forward-thinking clinician with a passion for caring for people, their families and carers.

Main duties of the job
The ideal candidate will be highly motivated and be able to demonstrate best practice in community mental health service delivery for older people, their families and carers. The ability to work independently, effectively and collaboratively within Multidisciplinary Teams, with external partners, and most importantly - with people, their families and unpaid or paid carers, is an essential core competency.

In the role you will be responsible for working with service users and their families. You will undertake a detailed history using a holistic approach and involve families and carers as part of our genuine commitment to co-creation. In this way you will promote positive experience and clinical outcomes from the beginning of the person's service journey.

There is a requirement to accurately record and document clinical discussions and assessment details on our clinical system and to ensure that all individual service user documentation is up to date and available to inform internal clinical meetings and aid consistent, safe and high-quality decision making by members of the MDT and wider colleagues.

About us

You will support the team's improvement activities for co-creation, aiming to deliver care that is highly personalised to each individual's needs and working closely with families, paid and unpaid carers. You will be encouraged and supported to bring your skills and experience to contribute to our team and locality's service development and continuous improvement and innovation endeavours.

Alongside the commitment from the Trust our team has a genuine sense of the importance of promoting staff well being. The Team has identified and proactive wellbeing champions, who aims to provide a range of opportunities for the team to think about how they stay well at work and in their personal lives.

We support and celebrate diversity, have active staff networks groups and are always looking at what more we can do to support our staff.

Job responsibilities
Please refer to the attached job description and person specification for further detailed information and to ensure that you meet the role criteria before applying.

We encourage interested candidates to make contact with us to find out more about our team and the services we provide.

You will have an enthusiastic approach towards your own personal and professional development, being open-minded to changes in the way that the pathway is delivered in order to support service improvement and effectiveness of delivery so that we can improve experiences of care.

In return, you will receive regular appraisal, clinical, management and caseload supervision and will benefit from the support of an experienced and committed multi-disciplinary leadership team, including the Team Manager and the Clinical Specialist Nurse/Advanced Nurse Practitioner.

Due to the nature of the role, it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.
