Job summary
Poole Central Primary Care Network are recruiting an experienced Paramedic or ANP / ACP to work in our Acute Home Visiting Team on a fixed term 1 year contract.

The successful candidate will be an experienced Paramedic, Advanced Nurse Practitioner (ANP), or Advanced Clinical Practitioner (ACP) who will provide advanced same-day clinical assessment, diagnosis and management of patients within primary care and community settings. The role will involve conducting visits in patients' own homes, delivering high-quality, patient-centred care whilst working autonomously and as part of the wider multidisciplinary team.

This role is up to 37.5 hours per week, worked Monday to Friday, excluding weekends and Bank Holidays. Working days and hours will be discussed at interview with shortlisted candidates.

Unfortunately we are not able to provide visa sponsorship for this role. Candidates must have the legal right to work in the UK.

Main duties of the job
The post holder will support on the day demands in General Practice by undertaking home visits to facilitate timely intervention for patients with acute health needs in the wider context of the patients health. This will involve a holistic assessment of their medical needs. The role will require the postholder to provide autonomous, advanced clinical assessment, diagnosis, treatment and management of patients requiring urgent or acute care within the community setting. This will require the post holder to work closely with the PCN and Practice Teams, as well as Community and Hospital based services and social care teams all serving the Poole Central population.

Applicants holding a First Contact Practitioner (FCP) qualification/accreditation are highly desirable. Independent Non-Medical Prescribing is advantageous.

Poole Central Primary Care Network is a group of 5 GP Practices in Poole, Dorset, covering a population of over 65,000. The practices work collaboratively to create joined-up, patient centred, community-based care and support for some of our most vulnerable patients and their carers. The successful candidate will work in the Acute Visiting Team alongside a team of experienced Paramedics and a Nurse Practitioner and the wider PCN team.

Job responsibilities
The post holder will be an experienced Advanced Clinical Practitioner ACP, Advanced Nurse Practitioner ANP or Registered Paramedic Practitioner who, acting within their professional boundaries, will provide advanced clinical assessment, diagnosis and management of patients presenting within the primary care and community setting.

The post holder will support the delivery of high-quality patient care and will contribute to service development and continuous improvement across the local primary care system, working collaboratively within the Acute Visiting Team, GP practices, community services and wider health and social care partners.

The post holder will demonstrate safe clinical decision-making and expert care for patients with acute, complex, frailty-related and undifferentiated health needs. Working autonomously and as part of a multidisciplinary team, they will assess, diagnose, treat, prescribe (where qualified), refer and/or signpost patients as appropriate and within their scope of practice and competence.

The post holder will work as an autonomous, accountable practitioner within the Acute Visiting Team, providing urgent same-day assessments and home visits for patients with acute illness, deteriorating health conditions, complex medical needs and frailty. The role will support admission avoidance and facilitate care closer to home wherever clinically appropriate.

The post holder may also be required to undertake Team Leader responsibilities, supporting the operational and clinical management of the Acute Visiting Team, workforce development, service improvement, and supervision of team members.

SCOPE AND PURPOSE OF ROLE
To work as an autonomous, accountable practitioner providing holistic assessment, diagnosis, treatment and ongoing management of patients with urgent and acute healthcare needs.

To provide high-quality clinical care within patients' homes, care homes and community settings, using advanced clinical skills and a broad evidence-based knowledge base.

To deliver same-day urgent care assessments and interventions that support admission avoidance and reduce unnecessary hospital attendance.

Manage an independent clinical caseload, ensuring patients receive timely access to appropriate care.

To work collaboratively with GPs, nurses, pharmacists, therapists, social care teams, community providers and secondary care services to deliver integrated care.

To support other members of the Acute Visiting Team through clinical advice, mentorship and professional leadership.

To undertake urgent home visits and care home visits in accordance with organisational protocols.

To assess, diagnose, treat, prescribe (where qualified), refer or signpost patients presenting with acute, undifferentiated, complex and frailty-related conditions.

To ensure effective risk management through appropriate assessment, incident reporting and risk mitigation.

To support the operational delivery of the service and undertake Team Leader responsibilities where required.

The post holder must own or have access to a suitable vehicle for undertaking community visits.

PRIMARY DUTIES AND AREAS OF RESPONSIBILITY
3.1 Clinical Role - The post-holder will:

• Provide a first point of contact within the Network for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan.
• Provide assessment, treatment and diagnosis at point of first contact, by attending to patients in a variety of clinical or non-clinical settings according to patients needs.
• Undertake home visits, in accordance with relevant PCN protocols.
• Make professionally autonomous decisions for which he/she is accountable.
• When appropriate, refer patients to an alternative care setting, ensuring relevant transfer arrangements are made.
• Instigate necessary invasive/non-invasive diagnostic tests/investigations and ensure relevant review of results are arranged.
• Ensure that professional standards are maintained in accordance with the HCPC (Health and Care Professions Council), Nursing and Midwifery Council (NMC) and clinical frameworks.
• Ensure that there is a robust system in place for maintaining clinical governance.
• Work with multi-disciplinary teams within the Network and across the broader healthcare community, to promote integrated and seamless pathways of care.
• Contribute to the Network achieving its quality targets to sustain the high standards of patient care and service delivery.
• Participate in the assessment of community health needs and help to develop patient and family-centred strategies to address them.

Help to develop and set up new patient services and participate in initiatives to improve existing patient services.

Support the review of emergency protocols and contribute to providing updates and training to staff, as appropriate.

3.2 Team Leader Responsibilities (where applicable) The post holder may be required to:

• Provide day-to-day clinical and operational leadership for the Acute Visiting Team.
• Support workforce planning and allocation of clinical resources.
• Act as a role model for advanced clinical practice and patient-centred care.
• Provide supervision, mentorship and support for junior clinicians and other team members.
• Support recruitment, induction, development and appraisal processes.
• Assist with rota management and service capacity planning.
• Escalate operational, workforce and patient safety concerns appropriately.
• Lead quality improvement projects and service development initiatives.
• Support monitoring of key performance indicators and service outcomes.
• Work collaboratively with senior leadership to develop and enhance the Acute Visiting Service.
• Promote a positive team culture focused on quality, safety and continuous improvement.

3.3 Teaching and mentoring role - The post-holder will desirably:
Promote a learning environment for patients, colleagues and other health professionals.

Contribute to the development and mentorship of for new and existing staff in the Network, including medical and nursing students, and PCN colleagues.

Share best practice and contribute to professional learning across the wider multidisciplinary team.

3.4 Professional role - The post-holder will:
Promote evidence-based practice using the latest research-based guidelines and the development of Network-based research.

Monitor the effectiveness of their own clinical practice through the quality assurance strategies, such as the use of audit and peer review.

Maintain relevant professional registration, such as with the HCPC, NMC.

Participate in continuing professional development (CPD) opportunities to ensure that up to date evidence-based knowledge and competence in all aspects of the role is maintained including maintaining up to date CPR training.

Record accurate consultation data within the patients records using agreed format.

Keep up to date with pertinent health-related policy and work with the Network and to consider the impact and possible strategies for implementation.

Work collaboratively with colleagues within and external to the Network.

Pro-actively promote the ANP / ACP, Paramedic role within the Network and externally to key stakeholders and agencies.

Identify personal learning needs and undertake development opportunities to improve service delivery.

3.5 Non-clinical team role - The post-holder will:
Identify appropriate opportunities to delegate both clinical and administrative tasks to other appropriate staff

Help the Network operate in a cost-effective manner.

Identify and manage care risks on a continuing basis.

Participate in Network meetings as required.

Participate in audits and inspections as appropriate.

Work closely with Network leadership team to seek to improve systems for monitoring/measuring performance against clinical governance and quality targets

Ensure all Network policies are fully implemented

• Knowledge/familiarity with medical terminology
