Job summary
We are looking for a motivated Advanced Nurse Practitioner or Advanced Clinical Practitioner to join our team.

This is an excellent opportunity to work in a practice with a diverse patient population and strong GP support.

Working alongside our existing team of GPs, ANPs and Paramedics, you will play a key role in delivering high-quality patient care through a combination of acute same-day clinics, home visits and community-based care.

We are seeking a confident, autonomous practitioner who enjoys a varied clinical workload, is comfortable managing acute presentations and is passionate about delivering accessible, patient-centered care.

Working Days: 34 days per week, with Wednesday and Friday essential. Flexibility is available for the remaining working day(s).

Main duties of the job
The successful candidate will work as part of our multidisciplinary team, delivering high-quality, patient-centred care across the practice, care homes and community settings.

Responsibilities may evolve in line with practice needs, and the following is not an exhaustive list.

The role includes:
  • Acute and Same-Day Clinics: Independently assess, diagnose and treat patients presenting with acute illness and urgent clinical needs.
  • Home Visiting: Assess and manage housebound patients, providing appropriate treatment, follow-up and ongoing care.
  • Clinical Triage: Assess urgent requests, including same-day home visits, and determine appropriate clinical management.
  • Care Home Rounds: Deliver regular care home rounds, working alongside the supervising GP and wider clinical team.
  • Clinical Assessment: Undertake comprehensive assessments, request investigations and develop treatment plans.
  • Independent Prescribing: Prescribe safely within clinical competence, following NICE guidance, local pathways and practice policies.
  • Care Planning: Support patients with frailty, complex needs and long-term conditions through personalised care planning.
  • Multidisciplinary Working: Collaborate with GPs, ANPs, Paramedics and community services to ensure coordinated care.
  • Clinical Governance: Maintain accurate records, provide safety-netting and escalate concerns appropriately.

About us
We are a well established GP practice providing high-quality care to a diverse local population. Our multidisciplinary team includes GPs, nurses, healthcare assistants, pharmacists, social prescribers, care coordinators and a skilled reception and administrative team, all working together to deliver safe, effective and patient-centred services.

We promote a professional, supportive and collaborative working environment where staff are valued and encouraged to contribute to service development. We are committed to high clinical standards, continuous improvement and staff wellbeing, and we actively support training, development and a healthy worklife balance.
