Job summary
A rare and exciting opportunity has arisen for an experienced, enthusiastic and motivated nurse to join our hard-working team. We are looking for Band 6 Community Health Nurse to join the Long Term Conditions team, you will be implementing the House of Care framework, providing holistic assessments within a patient's home.

The team are based at Brierley Hill Health and Social Care Centre and providing care across Dudley locality. The successful post holder will work autonomously as part of a highly motivated team of Case Managers providing a high standard of care and utilising holistic skills in the assessment, planning and implementing pro-active care needs to a population of patients with long term conditions in the community settings.

The candidate will have to be able to prioritise their own workload without supervision and be able to work well on their own initiative as well as part of a team.

Applicants will need to be flexible, approachable and have a sound clinical evidence-based background and knowledge. They will need to be friendly whilst assertive and be confident in their own decision making.

The service operates between the hours of 9am - 5pm, 7 days per week, 365 days per year. The successful applicant will be expected to work flexibly throughout this period. The service offers extensive support and knowledge to enable patients to be managed in the community, implementing the right level of care support to achieve this.

Main duties of the job
The post holder will work in collaboration with Primary and Secondary care clinicians, social care and mental health colleagues, to ensure care is patient centred and appropriate to their assessed needs.

Work in partnership with Dudley Group of Hospitals, Dudley Clinic Hub, Dudley Integrated Health and Care NHS and the Community Integrated Nursing teams to ascertain diagnosis, develop care plans, manage highly complex needs of the clients being assessed. Initiate follow up for those patient and co-ordinate the ongoing care with the ICT.

Using the knowledge and skills necessary to assess individuals and groups, identifying the multiple needs of the patient, family and carers giving in a holistic way taking into consideration cultural difference's.

Ability to verbally explain complex issues in formal situations such as court hearings / enquiries.

Negotiate and agree with patients, responsible clinicians and other care professional's individual roles and responsibilities with regard to action to be taken and outcomes to be achieved, referring to other services or professionals as appropriate.

Person Specification
Qualifications and Education
Essential
  • NMC Registered General /Adult Nurse.
  • Educated to diploma level.
  • Evidence of undertaking a recognised course of study in teaching and assessing
  • Evidence of professional development and regular study since qualification.

Desirable
  • Educated to first degree level or evidence of study at degree level.

Communication Skills
Essential
  • The ability to build effective relationships and act as an advocate for patients with Long Term Conditions.
  • Ability to manage patient information.
  • Ability to demonstrate negotiating and influencing skills.
  • Knowledge of clinical audit and effectiveness systems
  • Excellent IT skills.
  • Effective communication skills, including the ability to communicate highly sensitive issues and liaise with the key stake holders

Desirable
  • Experience of service planning and managing change.
  • Knowledge of effective people management practices

Knowledge
Essential
  • Evidence of high-level expert clinical skills in accordance with relevant clinical competencies.
  • Essential Evidence of teaching/ supervision skills
  • Knowledge of clinical governance, health and safety and risk management.
  • The ability to organise, prioritise and delegate duties and manage own workload.
  • Ability to reflect and critically appraise own performance.

Desirable
  • Ability to develop policies and guidelines to support provision of Long-Term Conditions.

Experience
Essential
  • Expertise or knowledge of House of Care Frameworks and how this is implemented within Long Term Conditions
  • Experience of supporting and developing junior members of staff and students
  • Evidence of continued professional development.
  • Experience of close liaison with members of multidisciplinary team.
  • Ability to communicate sensitive and emotional information to patients, their carers and NOK
  • Able to work flexibly e.g., support other Interview Nursing/AHP Teams/Services.

Planning Skills
Essential
  • Knowledge of current clinical development, evidence-based practices and professional issues affecting nursing.
  • Commitment to evidence-based practice.
  • Keen to develop own skills and develop nursing practice.
