We are looking for an enthusiastic Assistant Practitioner to join our Community Therapy Team within Rotherham NHS Foundation Trust. This team provides therapy and rehabilitation for adults with a very wide range of conditions in a predominantly community setting.

To be successful in this role you will be an experienced and competent individual with knowledge and enhanced skills of working in community rehabilitation within a healthcare setting, alongside MDT patient focused care and be able to demonstrate good communication skills.

You will join a team of existing Assistant Practitioners and Therapy Support Workers in our well-established service. You will support the Physiotherapists and Occupational Therapists in providing rehabilitation in the patients own home. The team provides a 7 day service.

We have a strong commitment to supervision, PDR and staff training/ development.

The successful applicant will have a full driving license with access to a vehicle for business use and a valid work permit to be eligible to work within the UK.

Main duties of the job
The role will specifically provide the following:

• Independently plan and provide programmes of therapeutic activity for patients, within protocols/procedures/guidelines; modifying within agreed parameters
• Implement and evaluate basic therapy interventions for patients relating to their treatment in line with specific condition pathways
• To assist the therapists with a range of duties and provide enhanced skills of direct care and support for patients
• To input accurate patient data onto the IT system
• To organise and plan own caseload and others to meet service and patient priorities. Readjusting plans as situations change/ arise in consultation with qualified staff where appropriate
• Leadership with delegation, support and supervision of therapy staff where required e.g. induction of new staff.

Person Specification
Experience
Essential

• Recent and relevant experience within an appropriate NHS setting or rehabilitation or re-ablement setting including community
• Experience of working within a multi-professional team

Qualifications
Essential

• Relevant foundation degree or NVQ level 3 in Diagnostic and Therapeutic Care or equivalent with additional relevant experience.
• Knowledge of specific patient conditions, forms of therapy acquired through training and experience.
• Previous significant experience as a therapy assistant or equivalent in a healthcare setting.
• Experience of working in the NHS

Knowledge
Essential

• Understanding of the role of the Assistant Practitioner within rehabilitation and its limitations
• Demonstrates knowledge of equipment available and able to assess and fit where applicable
• Able to recognise when guidance and support is required

Physical/Practical
Essential

• Due to the nature of the role it is essential that applicants are able to travel independently between TRFT sites and for patient visits in the Rotherham area and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.

Planning and Organising
Essential

• Demonstrates an ability to prioritise own, workload delegate and meet required timescales
• Responds appropriately to urgent and emergency situations
