The Thornton Practice is seeking Advanced Nurse Practitioners (ANPs), Advanced Clinical Practitioners (ACPs) and Paramedic Practitioners to join our Acute Infection Service, delivering same-day assessment and treatment for patients with respiratory infections, urinary tract infections, cellulitis and other acute infectious illnesses.

Working within a supportive, forward-thinking multidisciplinary team, you will use your advanced clinical skills to assess, diagnose, treat and prescribe for patients with urgent care needs, helping to improve access, avoid unnecessary hospital admissions and support local NHS services during periods of increased demand.

This role offers a high degree of autonomy, flexible sessional working, and the opportunity to work alongside experienced clinicians in a well-established, innovative practice serving over 13,000 patients. Main duties of the job
As part of our Winter Acute Infection Service, you will provide advanced clinical care to patients with urgent same-day healthcare needs. Working autonomously, you will assess patients, make clinical decisions, initiate treatment plans and prescribe where appropriate, ensuring patients receive timely, safe and effective care.

You will play a key role in supporting access to primary care by managing acute presentations, helping patients receive the right care in the right setting and reducing avoidable demand on urgent and emergency services. The role involves working closely with GPs and the wider multidisciplinary team, contributing to a coordinated and patient-centred approach to care.

In addition to providing direct patient care, you will contribute to clinical governance, quality improvement and service development activities, supporting the delivery of a high-quality and responsive service throughout the winter period. You will be expected to maintain accurate clinical records, follow evidence-based practice and help promote safe and effective use of medicines.

We require anywhere between 20-25 hours a week but we are very flexible.

Job Summary
The Acute Infection Advanced Practitioner is a pivotal member of the Winter Infection Service, providing high-quality clinical assessment, diagnosis, treatment and evaluation of patients presenting with acute infectious illnesses. Working autonomously and collaboratively with GPs, nurses, pharmacists, HCAs and wider multidisciplinary team colleagues, the practitioner will deliver safe, evidence-based care in line with their professional regulatory standards.

This role focuses on the assessment and management of acute respiratory infections, urinary tract infections, skin and soft tissue infections, and other same-day infectious presentations. The post holder will support admission avoidance, reduce unnecessary emergency department attendances, and contribute to the delivery of responsive winter services across the practice population.

Purpose of the job
The purpose of this role is to independently manage and provide advanced clinical care for patients presenting with acute infectious conditions, ensuring high-quality clinical decision-making and patient-centred care.

The Advanced Practitioner will:

• Deliver comprehensive clinical assessments, treatment plans, prescribing and follow-up.
• Manage patients presenting with respiratory, urinary and skin infections.
• Work collaboratively with the multidisciplinary team to support safe, efficient patient pathways.
• Contribute to clinical governance, quality improvement, audit and service development.
• Promote self-care, health education and antimicrobial stewardship.
• Support winter pressure initiatives and admission avoidance pathways.
• Uphold professional standards and engage in continuous professional development.

Key Responsibilities
Clinical Responsibilities
Undertake face-to-face consultations, telephone assessments and clinical triage.

Assess, diagnose, treat and manage acute infectious presentations.

Make autonomous clinical decisions regarding diagnosis and management.

Request, interpret and act upon relevant investigations and diagnostic tests.

Prescribe safely and effectively within scope of practice.

Manage patients presenting with:

• Acute upper and lower respiratory tract infections.
• Suspected pneumonia.
• Urinary tract infections.
• Skin and soft tissue infections, including cellulitis.
• Other acute infectious illnesses suitable for primary care management.
• Provide appropriate safety-netting and follow-up arrangements.
• Refer patients to secondary care or urgent services when clinically indicated.
• Maintain accurate and contemporaneous records within EMIS.
• Support admission avoidance and same-day urgent care pathways.

• Qualifications
• Registered General Nurse (NMC).
• Recognised Nurse Practitioner qualification at minimum degree level
• Independent Nurse Prescriber (V300).
• Teaching/mentoring experience or qualification.
