Job summary
The Woodberry Practice is looking for an enthusiastic and experienced Clinical Pharmacist to join our friendly and supportive primary care team.

This is a face-to-face, practice-based role at The Woodberry Practice, working directly with patients and alongside our GPs, pharmacy team and wider multidisciplinary team.

The successful candidate will play an important role in medicines optimisation and patient care, helping to ensure patients receive safe, effective and appropriate treatment. Previous experience working within a GP surgery or primary care setting and experience using EMIS are essential for this role.

We are looking for someone who is organised, proactive and confident working independently as well as collaboratively. You will have excellent communication skills, a patient-centred approach and the ability to effectively manage your own clinical workload.

Main duties of the job
The Clinical Pharmacist will provide face-to-face clinical pharmacy support to patients at The Woodberry Practice, working closely with GPs and the wider practice team.

The role will include undertaking structured medication reviews, supporting patients with long-term conditions, medicines optimisation, medicines reconciliation following hospital discharge and responding to medication and prescribing queries.

You will support safe and effective prescribing, identify and address medication-related risks, ensure appropriate medication monitoring is completed and provide advice to patients and healthcare professionals.

The role will also involve reviewing clinical information, using EMIS and relevant clinical systems, and contributing to prescribing, patient safety and quality improvement work within the practice.

You will be expected to manage your workload effectively while working collaboratively with the wider team to support high-quality patient care.

Job responsibilities
Main Duties and Responsibilities

• Undertake structured medication reviews, particularly for patients with complex medication needs, polypharmacy or long-term conditions.
• Provide face-to-face patient consultations to review medicines, adherence, effectiveness, side effects and monitoring requirements.
• Support patients with the management of long-term conditions through medicines optimisation and appropriate clinical advice.
• Resolve day-to-day medicines-related and prescribing issues within the practice.
• Respond to medication and prescribing queries from patients and members of the practice team.
• Promote safe, effective and evidence-based prescribing.
• Support repeat prescribing processes and appropriate medication monitoring.
• Undertake medicines reconciliation following discharge from hospital or other care settings.
• Identify medication discrepancies, interactions, duplications and other medicines-related risks and take appropriate action.
• Manage medicines shortages and supply issues, recommending suitable alternatives where clinically appropriate.
• Support effective communication across patients' care pathways and liaise with hospitals, community pharmacies and other healthcare professionals when required.
• Support the monitoring of patients taking high-risk medicines.
• Contribute to medication safety alerts, prescribing reviews, clinical audits and quality improvement work.
• Work collaboratively with GPs, nurses, pharmacists, pharmacy technicians and the wider practice team.
• Use EMIS and other relevant clinical systems effectively and maintain clear and accurate patient records.
• Prescribe medicines within professional competence, in accordance with the postholder's Independent Prescriber qualification, current clinical guidance and agreed practice arrangements.

Professional Responsibilities
The postholder will maintain registration with the General Pharmaceutical Council (GPhC), work within their professional competence and maintain appropriate continuing professional development.

They will comply with practice policies, clinical governance, confidentiality, information governance and data protection requirements and contribute positively to the wider practice team.

Essential

• Experience working as a Clinical Pharmacist within a GP surgery / primary care setting
• Experience using EMIS in a GP practice / primary care setting
• Experience providing face-to-face patient consultations
• Experience undertaking medication reviews and supporting patients with long-term conditions and polypharmacy
• Experience working effectively with GPs and the wider practice team
• Experience managing medicines-related queries and medicines reconciliation

Desirable

• Experience of clinical audit and improving patient care

Qualifications
Essential

• Master's degree in Pharmacy (MPharm) or equivalent pharmacy qualification
• Registered Pharmacist with the General Pharmaceutical Council (GPhC)
• Independent Prescribing qualification with current annotation on the GPhC register
• Evidence of commitment to ongoing personal and professional development

Desirable

• Medicines Management qualification
• Minor Ailments certification
• Relevant postgraduate clinical pharmacy qualification
• Additional training relevant to primary care / general practice
