Job summaryWe are seeking a Clinical Pharmacist to join our team at Richmond Clinic, supporting prescribing activity in general practice with a significant role in medication reviews.

Main duties of the job- Conducting comprehensive medication reviews and optimising medication regimens for patients with complex medical needs.
- Providing advice on medication management to patients, healthcare professionals, and the multidisciplinary team.
- Supporting the management of chronic diseases through medication adjustments and monitoring after discussion with medical team.
- Corresponding with secondary teams if clarification needed of prescribing plans.
About usRichmond Clinic is conveniently located on the B4596, the main road from Junction 25 (M4) towards Newport Centre. We are very close to all amenities (dentist, chemist, optician, banks etc) and the bus stop is directly outside the surgery.
The practice consists of 3 GP Partners, 2 salaried GP's 2 Nurses and a Phlebotomist. we have 6 receptionists, 2 Admin Clerks, Deputy Practice Manager and Practice Manager. We have just over 6K patients.

QualificationsEssential- Registered Pharmacist with current GPhC registration.
- Proven experience as a Clinical Pharmacist, preferably in a primary care or multidisciplinary setting.
- A kind, caring, patient focussed approach
- Basic clinical skills to assist with review of medication (e.g. blood pressure check)
- Strong communication and interpersonal skills, with an ability to work effectively as part of a multidisciplinary team.
Desirable- Registered Pharmacist with current GPhC registration.
- Proven experience as a Clinical Pharmacist, preferably in a primary care or multidisciplinary setting.
- A kind, caring, patient focussed approach
