We have an exciting opportunity to support some of the most vulnerable patients within our healthcare system.

Based at UCLH and across NCL, you'll be part of a small, dedicated homeless intermediate care team working across our NCL acute hospital trusts, P2 beds and community to support patients experiencing homelessness and social exclusion.

You will play a key role in helping patients navigate their health and discharge journey from admission to discharge and in community follow up working closely with clinical teams, local authorities, and community services to ensure safe, appropriate, and person-centred transitions from hospital to community care.

You will also line manage a team of clinicians and non-clinicians in supporting our patients, focusing on staff improvement and wellbeing as well as continuous service improvement for our patients.

Main duties of the job

• Line managing a team of clinicians and non-clinicians in the homeless intermediate care team
• Support the band 8a clinical operations manager in service improvement and continuous development of the team.
• Work with the NCL acute hospital trusts in supporting homeless patients in their journey from admission to discharge
• Education/teaching to acute trusts about homelessness and the procedures from hospital
• Manage a caseload of complex patients both in hospital and in the community across NCL

Person Specification
Knowledge and Qualifications
Essential

• NMC adult registered nurse
• Knowledge of the particular issues that characterise the provision of health services to vulnerable groups and homeless persons

Desirable

• Leadership qualification
• Service development and quality improvement

Experience
Essential

• Substantial experience managing complex caseloads of a team
• Experience working in the homelessness sector
• Experience of line management and leadership of teams and services

Desirable

• Demonstrate knowledge of complex discharge procedures and flow
• Experience working with people with co-occurring mental health and substance misuse problems and physical health
