This post is pivotal both within the pharmacy department and across the wider Trust and system.
For the successful candidate this will provide the opportunity to lead in an innovative way with autonomy and support, to help shape the future of medicines access to patients at UHCW.
Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.Main duties of the jobThe primary role of this specialist post is to support the effective, decision making and management of the medicines formulary across UHCW.
This will include the introduction of new medications, audit / management of existing practice and joint work across the interface with primary care colleagues to ensure the cost effective evidence based use of medicines across the system.
To scope, lead and facilitate on agreed projects affecting medicines across the primary / secondary care interface to ensure best value and practice for the NHS.
The post holder will provide expert support to healthcare professionals to align to our robust medicines optimisation principals upon the development and introduction of new medications both within UHCW and across the wider system.

Job responsibilities- Review outcomes of the Area Prescribing Committee and work with secondary care consultants and pharmacists to implement, supporting the changeover of treatment for patients to formulary choices where appropriate.
- To support Trust clinicians in the preparation of APC applications.
- To work with GPs and Practice Pharmacy teams to support changes in formulary or education regarding drugs within secondary care practice.
- Lead the proactive review and management of the formulary from a secondary care perspective, supporting prescribing decision making.
- To contribute and lead where appropriate, in the delivery of WRP and cost-effective prescribing appropriate to this role and in both primary and secondary care.
- Lead on interface projects which look to improve cost effectiveness and/or safety of prescribing.
- Review and maintain the management of shared care protocols where appropriate. Lead on resolution of GP refusals, ensuring patients receive their medicines in a timely manner with the most appropriate clinician maintaining responsibility for their prescription and care. Co-ordinate this activity with the Pharmacy Homecare Team where appropriate and the High Cost Drugs Team in order to make sure the medication is charged to the correct budget.
- Co-ordinate subgroups of the APC to develop specific guidelines ensuring that relevant stakeholders are included. Where possible, ensure that guidelines are produced that are consistent and relevant in both primary and secondary care.
- Liaise with other senior pharmacists within partner organisation (including any outsourced secondary care pharmacy services) to ensure delivery of the health communitys objectives and targets. Support primary and secondary care pharmacists to implement the activities of the Coventry and Warwickshire Area Prescribing Committee.
- Respond to interface enquiries from healthcare professionals, staff, patients and public within the Coventry and Warwickshire area. This may include liaison work with the ICB Medicines Optimisation team.
This role will be responsible for managing and maintaining the cost effective introduction and use of medications across UHCW and the wider system.

- + 1 year trainee pharmacist training (or equivalent)
- Registered as a practising pharmacist with the GPhC
- Post-graduate Diploma in Clinical Pharmacy or equivalent experience
- Evidence of CPD
Desirable- Member of a professional organisation (eg Royal College of Pharmacy)
- Pharmacy or Pharmacy Practice related MSc
- Independent Prescriber
ExperienceEssential- Suitable experience dealing with complex clinical situations in hospital
- Suitable and relevant hospital pharmacy experience post registration
- Experienced at working with and influencing senior medical and nursing staff
Desirable- Experience at a directorate pharmacist level.
- Experience in line managing staff and conducting appraisals.
KnowledgeEssential- Good sound clinical pharmacy knowledge
- Up-to-date knowledge of Medicines legislation
- An understanding on how medications are introduced into a hospital.
Desirable- Good specialist knowledge of areas of clinical responsibility
