Job summary
With frailty and an ageing population at the forefront of service development across the NHS, we are looking for an enthusiastic and motivated Physiotherapist with a passion for supporting older adults within an acute setting to join our dedicated Frailty rotation which spans our Care of the Elderly and Medicine wards.

Our Care of the Elderly service covers four wards, providing specialist care for older adults with:

• Delirium
• Falls
• Dementia
• Infections

Our Medicine service covers four wards, providing specialist care for adults, with:

• Respiratory conditions
• Long-term conditions
• Endocrinology conditions

The patients we support are often living with frailty and are at significant risk of deconditioning during their hospital stay. If you have broad inpatient experience and a particular interest in frailty management, rehabilitation, and preventing deconditioning, this could be the ideal role for you.

As a Band 6 Physiotherapist joining the team, you will benefit from a two-week supernumerary induction period, giving you the opportunity to become familiar with the service, meet the wider team, and identify any development or training needs. We are committed to supporting your professional growth and helping you thrive in your new role.

Main duties of the job

• To manage a caseload of patients with varying diagnosis and / or complex needs, using evidence-based practice with client centred principles, to assess, plan, implement and evaluate interventions
• To support the learning and development of students and staff
• To develop enhanced clinical skills and knowledge in an area of interest linked with patient cohorts (e.g. cognition, specific clinical condition, prehabilitation / acute rehabilitation)
• To participate in 7-day working patterns as required
• To participate in the respiratory on-call service

Job responsibilities

• Be professional, legally responsible & accountable for all aspects of own work and activities, to manage clinical risk within own clinical caseload.
• Carry a clinical caseload of highly complex patients, provide advanced therapeutic assessment & evaluation and provide detailed written treatment plans with patient centered goals working within ward / department areas.
• Work as an autonomous practitioner and member of the multidisciplinary team, contributing to team discussions and influencing decisions about patient care programs, taking on the role of care coordinator where relevant.
• Carry out treatment, using a broad variety of modalities and clinical reasoning to allow selection of the most appropriate technique evaluating, modifying and recording all interventions and outcomes & adapting them to meet the needs of the patient.
• Use highly developed manual treatment skills requiring coordination, sensation & dexterity.
• Work in accordance with CSP & HCPC rules of professional conduct and within local care pathways, policies and standards.
• Establish and maintain effective communication networks with patients, carers, MDT, other workers and agencies across health, social care and the private sector locally and regionally.
• Contribute to the Group and Trust clinical governance arrangements and quality agenda, including the setting and monitoring of practice standards, clinical risks, and Health & Safety.
• Take the lead for information management within area of responsibility to ensure high quality up to date written and electronic records and activity data are maintained in accordance with Professional and Trust standards & produce in a timely manner
• Be aware of and assist with managing resources and equipment necessary to meet service needs in clinical area reporting any faults.

• BSc / MSc Physiotherapy and evidence of post graduate training in relevant specialist area
• HCPC registered
• Clinical education training or willingness to complete within one year

Experience

• Current (or recent) and substantial working experience of specific /specified area or relevant associated area
• Significant evidence of treating patients who are acutely unwell including respiratory management and treating patients at Critical Care level
• Current or recent experience of participation in respiratory on call rota
• Documented evidence of continuing professional development.
• MDT Working.
