Job summaryThis role is to cover maternity leave until 30/08/2027. Working 22.5 hours per week, the shift pattern is over 7 days a week and will include regular evening shifts and may be required to cover night shifts to cover sickness and annual leave as the need arises.
The post holders will work as a registered Nurse within Community Services across the Dalton and Ulverston District nursing footprint, accountable for providing evidence based, holistic nursing care of patients.
The post holders will be required to work as part of the integrated team across community settings such as patients' homes, residential and nursing homes. Flexible working will be required to meet the needs of the patients group and the service as it continues to develop. Whilst working with other health care professionals the post holder will develop the ethos of integrated care and seamless services ensuring that patient care is delivered to the highest possible standard.
You will be expected to support the Sister/Team Leader in leading the team supervising, directing staff and delegating workload according to skills of the staff within the team.

Community nurses deliver a wife range of highly skilled interventions in the patients own home. This includes the management of long term conditions including diabetes care, end of life care, catheter care and symptom control and support. We support patients to remain in their own homes for as long as possible, preventing hospital admission and supporting patients to lead a long and happy life.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies,daycase and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
Our Community Service for adults are provided in people's homes ,community centres, clinics, GP Practices, community hospitals. Our aim is to work with people to help them remain independent , improve their health and manage their conditions through high quality care , advice and support.

Good communication skills and a solid documentation skills set across a range of IT systems is required. You will poses a wide range of clinical skills such as female/male and suprapubic catheterisation, compression bandaging, doppler investigations, IPC drain care, venepuncture, a wide range of wound care, medications administration, syringe pump management, bowel and bladder assessment and care and much more.
You will work predominantly as a lone worker, making autonomous decisions and have the support and contact of your team throughout the shift. Use of a car is required.

Desirable- Community Specialist Practitioners Qualification
- NMP
- Supporting professionals in rpactice/mentoring
ExperienceEssential- Recent nursing experience
- Experience of lone working.
- Demonstrate experience of delivering high quality personalised care.
- Demonstrate evidence to work as part of a team.
- Demonstrable experience of using initiative and effective prioritisation
