Job summary
This is part of Acute Care at Home Service (ACAHS) which is responsible for delivering in the Community 2hr urgent care response via Community Rapid Intervention Service (CRIS), Virtual Wards and Acute Visiting Service. Ultimate aim is admission avoidance and early supported discharge to enable patients that would benefit from acute care within their own home. The ACAH has Medical governance with a consultant of the week covering the 3 service areas. Geographical area covers both Royal Stoke and County Hospital with patients having a GP in Staffordshire.

Main duties of the job
The consultant will be part of a 1 in 4 week rota providing Consultant of the Week for the service along with 2 other consultants. This is managed as 2PA within job planning.
The post will conduct daily ward rounds and discuss patients under their responsibility. Providing decision-making, advice and support to ensure safe and effective care

Job responsibilities
  • Provision of cover to the CRIS and Virtual Wards, attendance at daily ward rounds
  • Provide advice and support to clinical staff
  • Provide cross cover when colleagues are on leave
  • Take on-going responsibility for patients under his/her care
  • Participate in the development of protocols and guidelines
  • Take an appropriate share of responsibility for the professional supervision and development of clinical colleagues with the speciality
  • Demonstrate a firm involvement in Clinical Governance, Risk management and Clinical Audit

Person Specification
Qualifications
Essential
  • Entry on the GMC Specialist Register in General Internal Medicine

Desirable
  • MD, PHD or equivalent

Knowledge, Skills, Training and Experience
Essential
  • Clinical Training and experience equivalent to that required for gaining UK CCT in general Interal medicine
  • Ability to offer expert clinical opinion in a range of problems with specialitities
  • Ability to take on full and independent responsibility for the care of patients
  • Thorough and detailed knowledge of medical audit, medical educations, current clinical and medical best practice
  • Understanding of Clinical Governance process
  • Ability to organise, manage, prioritise waiting list
  • Experience of conducting clinical audits

Desirable
  • Evidence of ability to teach and experience of teaching

Personal Qualities
Essential
  • Enthusiasm and the ability to work under pressure
  • An enquiring and critical approach to work
  • Caring attitude to patients
  • Ability to communicate effectively with colleagues, patients, relatives, GPs, nurses and other staff
  • To be accessible for on call
