We are looking for an enthusiastic and compassionate Occupational Therapist who is passionate about recovery-focused person-centred care to join our Mental Health Integrated Network Team (MINT) providing community based care in Ealing and Southall. This is an exciting opportunity for an Occupational Therapist looking to develop into a confident senior clinician while making a meaningful difference to the lives of people experiencing mental health challenges.

You will work alongside experienced multidisciplinary teams, managing your own clinical caseload while developing specialist skills in assessment, intervention and recovery-focused practice.

Main duties of the job
You will manage a varied clinical caseload of adults aged 18-65 with a range of mental health conditions, delivering high-quality, person-centred occupational therapy and care coordination interventions that promote recovery, independence and meaningful participation in daily life.

Your key responsibilities will include:

• Managing a defined caseload, undertaking comprehensive occupational therapy assessments and developing evidence-based, client-centred intervention plans using sound clinical reasoning.
• Completing initial assessments, contributing to risk assessments, crisis plans and care plans, and participating in ongoing care coordination where appropriate.
• Supporting service users and their carers to develop personalised goals and implementing trauma informed approaches that maximise occupational performance and social inclusion.
• Planning and facilitating safe, timely discharge and transfer of care to primary care, community services and other appropriate agencies.
• Working closely with partner organisations to ensure continuity of care and effective transitions between services.
• Providing clinical supervision, support and guidance to junior occupational therapists, occupational therapy assistants and support staff, contributing to their professional development.
• Participating in service evaluation, clinical audit and quality improvement initiatives, using evidence-based practice to support ongoing service development.
