Job summary
We are seeking a compassionate, enthusiastic Advanced Nurse Practitioner (ANP) to join our friendly and supportive team at West Quay Medical Practice. The candidate will have previous experience in general practice.

This role is suitable for both experienced ACPs and those who have recently qualified.

The successful candidate will deliver high-quality, patient-centred care, working autonomously to assess, diagnose, treat, and manage a broad range of acute, minor, and long-term conditions. You will manage patients presenting with undifferentiated, undiagnosed issues carrying out full clinical assessments, initiating treatment plans, prescribing, and evaluating care in line with national guidelines.

You will also contribute to the ongoing development and innovation of the practice, playing an active role in service improvement and patient care delivery.

Main duties of the job
  • Provide expert clinical assessment and diagnosis for patients presenting with undifferentiated, undiagnosed, and complex health problems.
  • Initiate, evaluate, and review treatment plans, including prescribing medications and therapies within your scope of practice.
  • Manage a range of acute, minor, and long-term conditions independently, referring to or liaising with GPs or secondary care where appropriate.
  • Undertake comprehensive physical assessments, using advanced clinical skills and decision-making.
  • Perform and interpret relevant investigations, such as blood tests, ECGs, and other diagnostic procedures.
  • Promote preventative healthcare and offer patient education, self-care advice, and health promotion.
  • Work collaboratively with the multidisciplinary team to ensure high standards of care, continuity, and efficiency.
  • Contribute to clinical governance, audits, significant event analysis, and the development of local policies and protocols.
  • Participate in service development and innovation, supporting quality improvement initiatives across the practice.
  • Maintain accurate and timely patient records in accordance with legal, ethical, and professional standards.
  • Work within local policies, procedures and guidelines.

About us
We are a practice of around 6200 patients, with 2 GP Partners, 2 Salaried GPs, 3 Practice Nurses and a small team of Healthcare Assistants and Phlebotomists. We have an experienced and friendly admin team who support the clinical staff with the everyday running of the practice.

We are also supported by the local Primary Care Network and have access through this to Mental Health Practitioners, First Contact Practitioners and a team of Pharmacists.

Job responsibilities
The Advanced Nurse Practitioner (ANP) will have expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients.

The ANP provides an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment.

ANPs in primary care can develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. They are autonomous in making decisions based on assessment, diagnosis and interpretation of test results. They can independently prescribe appropriate medication, evaluate or refer to other specialists if necessary.

Experience
Essential
  • Experience of practice within the four pillars
  • Job plan that demonstrates advanced nursing practice and has equity with peers working at this level
  • Experience of prescribing and undertaking medication reviews

Desirable
  • Experience of working in a primary care environment

Qualifications
Essential
  • Registered Nurse with Nursing and Midwifery Council
  • Masters degree required for qualification post December 2020 refer to RCN Credentialing for Advanced Level of Nursing Practice.
  • or
  • Post graduate diploma or degree for Advanced Practice Qualification up to December 2020
  • Qualified Independent Nurse Prescriber on the NMC register
  • Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet
  • Meets the standards for registered ANP working at advanced level

Knowledge & Skills

  • Ability to work within own scope of practice and understand when to refer to GPs
  • Knowledge of health promotion strategies
  • Understand the requirement for PGDs and associated policy
  • Polite and confident, flexible and cooperative

  • Problem solver with the ability to process information accurately and effectively, interpreting data as required
  • High levels of integrity and loyalty
  • Sensitive and empathetic in distressing situations as well as ability to work under pressure/in stressful situations
