Job SummaryWestern Road Surgery is looking for a Practice Nurse to cover Maternity Leave. The role will be for 25.5 hours over 3 days, Monday - Wednesday. As part of the team, you will be helping the surgery achieve QOF targets, Cervical Cytology, Wound Care and more. The role ranges from treating patients in nurse-led clinics to utilising call and recall systems to deliver high quality patient care. The surgery has 13,000 patients so is very fast paced. The post-holder will work with reference to Practice and national protocols and policies, they will also have the required level of training and competence to carry out this role.
Main Duties of the Job- Management of Chronic Disease
- Identifying significant abnormalities and referring appropriately
- Monitor and development individual management plans for patients with chronic diseases and agreeing these as appropriate with the patient - Patient Health Checks
- Identifying significant abnormalities and referring appropriately
- Obtaining appropriate lifestyle information, identifying appropriate health promotion issues and working in collaboration with local providers to reduce health inequalities - Health Screening
- Carry out Cervical Cytology
- Carry out 24 hour ECGs and BPs, routine ECGs and vaccinations - Other
- Child and Adult Immunisations
- Completing wound care for patients - Audit
- Support the Nursing Team with Infection Prevention Control (IPC) by providing audits, training, support and evidence of compliance to the practice manager
