Job Overview
Are you an experienced and compassionate Registered Nurse with a passion for palliative and end-of-life care? We are seeking a Clinical Nurse Specialist to join our Community Palliative Care Team, delivering high-quality care to patients and families across Northwest Surrey.

This is an exciting opportunity to play a key role in the delivery of community-based palliative and end-of-life care, managing a complex caseload and providing expert clinical assessment, symptom management and care coordination. Working autonomously and as part of a multidisciplinary team, you will support patients and families through some of the most challenging times in their lives, helping them to achieve their preferred place of care wherever possible.

As a Clinical Nurse Specialist, you will also provide clinical leadership within the team, support service development and contribute to the ongoing improvement of patient care and service delivery.

Main Duties of the Role
In this role, you will manage a community caseload of patients with complex palliative and end-of-life care needs, undertaking holistic assessments, developing and implementing care plans, and providing expert symptom management and supportive care.

You will work closely with GPs, Community Nursing Teams, acute hospitals, social care providers, care homes and other healthcare professionals to coordinate seamless, person-centred care. You will act as an advocate for patients and families, supporting advance care planning, complex decision-making and access to appropriate services.

The role also includes providing clinical leadership and mentorship to colleagues, contributing to multidisciplinary team discussions, supporting quality improvement initiatives, participating in education and training activities, and helping to drive service development across the Community Palliative Care Team.

About You
We are looking for an experienced Registered Nurse with excellent clinical assessment, communication and decision-making skills, and a commitment to delivering compassionate, high-quality palliative and end-of-life care.

You will have experience caring for patients with complex needs and be confident managing a caseload autonomously whilst working collaboratively within a multidisciplinary team. You will be able to communicate sensitive and complex information with empathy and professionalism and build effective relationships with patients, families, carers and healthcare colleagues.

Experience within palliative care, oncology, community nursing or a related speciality is essential. You will also demonstrate leadership qualities, a commitment to continuous professional development and a passion for improving patient care and service delivery.

Additional Information
The successful candidate will be required to have the legal right to work in the UK, maintain professional registration, complete relevant mandatory training and undergo an appropriate DBS check.

A full UK driving licence and access to a vehicle for work purposes are essential due to the community-based nature of the role.

The postholder will be expected to participate in the delivery of a seven-day service and contribute to the ongoing development of the Community Palliative Care Service.
