We are looking for a dynamic registered practitioner with advanced clinical skills. Central Norfolk IOPS Team provides a 7 Day a week, 0800-2100hrs service currently . You will be responsible for supporting the team with triage, providing advice to primary care clinicians where appropriate, You will be required to prescribe and your scope of prescribing practice must include prescribing in Older Persons Mental Health. You will also be required to support the wider team with assessments and treatment planning, aiding the team with risk management and co-ordination of care delivery within a Crisis service. You will be supported with regular supervision and opportunities to work in partnership with other health and social care providers, linking into the wider multi-disciplinary service.

Main duties of the job
The Clinical Nurse Specialist will provide expert clinical leadership and specialist assessment for older people experiencing mental health crisis, supporting the team's admission avoidance and home treatment functions.

Key responsibilities include:

• Undertaking complex mental health, risk and capacity assessments.
• Providing specialist interventions for older adults with dementia, delirium, depression, psychosis and other acute mental health needs.
• Supporting crisis resolution and admission avoidance, enabling people to remain safely in their own home or care setting where appropriate.
• Acting as a senior clinical resource within the multidisciplinary team, advising on complex cases and contributing to clinical decision-making.
• Supporting effective partnership working with families, carers, primary care, acute hospitals, care homes and social care services.
• Providing clinical supervision, training and development to enhance workforce capability and confidence.
• Leading on specialist practice relating to older people's mental health, dementia care and crisis management.
• Contributing to service development, quality improvement and transformation initiatives to strengthen community-based crisis care.
• Supporting the Trust's objectives around reducing inpatient admissions, improving patient flow, and delivering equitable multidisciplinary services for older people.

• RMN or equivalent professional qualification
• Non-Medical Independent Prescriber
• Degree Level Qualification or equivalent or equivalent level

• Study experience at Masters level, study at that level or clinical experience
